The exact cause can be determined only after a complete medical history and clinical examination. In Balanoposthitis there is inflammation of the glans (glands) penis. There is pain swelling and inability to retract the foreskin. A complication of balanitis in recurrent cases is constricting phimosis, or inability to retract the foreskin from the glans (glands) penis .This can occur due to collection of smegma and lack of hygiene. Since you are in severe pain you need immediate medical care. Consult an urologist and get the diagnosis confirmed. Topical antibiotics, mild steroids and circumcision may be needed depending on the severity of infection. This is only a possibility. An examination shall confirm the exact cause.
Best of luck and regards!
