recurring round, itchy spots on face

It began as one spot on my forehead that I took to be a spider bite: large, round, slightly raised red bump that was tender. Around the same time I had a nasal infection for which my doctor prescribed antibiotics and both things improved. About a month later, on the first day of my period, the spot on my forehead flared up again along with four additional spots on my chin. The spot on my forehead looked a little like ringworm, so my doctor prescribed broad-spectrum antibiotics and an anti-fungal cream, and advised me to make an appointment with a dermatologist. I couldn't get an appointment for two weeks, by which time the condition had improved to where the spots were no longer inflamed and had diminished and faded but not entirely disappeared, but it looked merely like minor residual scarring. A month later, on the first day of my period again, the spots flared up. I decided to let it go a little and see if it cleared up on it's own, but by the second day the spots began to blister, so I went to my doctor again. Again, he prescribed the broad-spectrum antibiotics for two weeks, cultured my nose, and gave me a prescription for antibiotic ointment to put in my nose. I also saw a dermatologist the following day, but he had nothing more to add to my primary doctor's diagnosis of a staph infection, except to say that he wished he'd been able to culture the lesions before the antibiotics kicked in. Two days ago I got my period and my same spots flared up again. The on-call dermatologist said that I could wait it out to see if it blistered again, said they can't do a culture unless it's "weepy," or I could start on antibiotics again. I opted to start doxycycline, since while I'd certainly like a definitive answer, each time this condition flares up the spots get bigger and they leave marks.

My doctors completely discount the fact that the condition always flares up on the first day of my period. They have stuck with the idea that this is a staph infection, even though the culture from my nose turned up staph but nothing drug-resistant, and presumably this should have been cleared up by the ointment. I've been washing both my hands and face with antibacterial soap this entire past month, and this month I refrained from taking any pain killers for my period just in case there was some allergic tie-in with the naproxen I've taken in the past.

The spots are very itchy and painful, but at least this time there's no blistering. I also seem to be getting a lesser rash simultaneously on my neck, particularly behind my ears, but it's less persistent and doesn't have the clearly-defined spots. I'm attaching a photo of the single forehead spot, this forum seems to allow only one picture, from one day after beginning the doxycycline, the spots aren't as livid as they were yesterday.

hi ! i have 2 recurring spots same description as yours,  as well as the nasal "infection" . its been 5 months now, no help from doctors or dermatologist! Were you ever able to find out what it was ?
