Penis skin irritation w/ stretched skin?

Hello, for the past few months I've been having some slight pain directly under my penis head and some change in skin texture.  I've noticed that there are almost "holes" forming in the skin from what I think is from excessive masturbation from the skin constantly getting tugged.  I am circumcised, 26, and have been in a monogamous relationship for 4 years now, so I'm not worried that it's an STD or anything related to such.  I always use lubrication when masturbating, and the area only seems to be sore after sex or masturbation.  I have gone a few days without such and have been applying the penile lotion Man1Man oil and the pain is gone, but the skin texture is still the same, without any noticeable change.  Is this how my penis is going to look now forever or will it eventually heal? Is this even from masturbation?  Thank you in advance!
