stubborn spreading fungus

There is a bit of history with this so please bear with me. A good while back i went to take a shower in my bathroom. My little toe and only my little toe stepped into a green like fungus in the tub. After that my little toe got itchy, purple spots began  to show up and then the blisters came. I went and had it treated with ketoconicale and some pills and that seemed to take care of it.

Eventually however it came back and spread to the other toes. I kept trying new creams and pills but no change. In fact the creams seem to spread the thing even more to the point where i am at now. It now cover all my toes on the left foot and the bottom of my foot. It has spread to my other foot, my hands. The pain i get from it now is that it drys up and cracks, bleeding so i can't even put the cream on it if i wanted to. The doc finally broke down and told me that the fungus will attempt to spread when under attack by the cream sending out spores and that is why it appears in new places. Yet no progress has been made in having it receed. What can i do? I did al the basics from over the counter to changing shoes, etc. I don't want to use more cream if it just spreads it more, i already freak people out. Help!!!
