Sometimes too few carbs can actually cause trouble too. I learned this the hard way. Was eating less and less and still BG higher - liver is other source and will do if we don't get from nutrition. Also, sleep apnea very common with diabetic population. Strongly suggest you talk to Dr and ask to see diabetic educator - they did wonders for me!!!
I am on Janvia but I have not seen a change. I will try than 20 carbs a meal. I don't snack.
40 gr of carbs a meal is still 160 gr a day plus snack.
you already know that carbs raise BG carba cause BG swings.
I eat ~50gr of carbs a DAY.
You may need more help than just Metformin, you might need insulin. Met does not help with countering the BG rise from eating carbs for that you need insulin. A fast acting insulin.
Oh forgot a1c started at 6.9 currently 5.9...but spikes are thethe issue
I'm sensitive to the slightest blood sugar change. Fasting I run 115, meals 150...but I have that falling asleep and can't fight it if my sugar is above 115. I can't eat any fruit or bread without the resulting high. I just feel horrible and find g something to eat is a chore... Everything causes the unwanted swing.

would suggest you get your thyroid tested, as low thyroid is also fairly common in people with diabetes.
How well controlled are your blood sugars? What is your hba1c?
If your blood sugars are still high (and considering you are having ibs issues), you maybe should ask your dr about starting on insulin.
