swollen gland in neck

our 10 year old doxie has been diagnosed with congestive heart failure and is on medication and doing ok, but the other day I noticed a gland in his neck was swollen the size of a golf ball. He is not eating very much these days I think mostly due to the medicine but any thoughts on this

With such a delicate CHF situation and the meds at hand, I would most definetly always try to consult w/vet over and over for answers....I am not familiar with these symptoms, but maybe some others will post comments who have experience such..
