The numerator refers to the distance of the patient from the eye chart, usually 20 feet in the U.S. while the denominator refers to theability to resolve a character which subtends 5 minute of arc. The larger the denominator, the larger size letter able to be resolved.

Pretend you have a target. Now place the target 50 feet away. A person who has 20/20 vision would be able to see the target from 50 feet away. However, a person with 20/50 vision would have to move until s/he is standing only 20 feet away from the target.
Basically 20/20 vision is "perfect" visual acuity. A person with 20/20 will be able to see all things clearly within 20 feet. The higher the bottom number is the worse a person's vision is. There are also some people who have better than 20/20.
Also, keep in mind most people are able to have 20/20 vision with the use of glasses or contacts.
