Strange "ball" on elbow

Hello,
this post is for helping a friend of mine.
She suffer for a long time of anxiety, panic attack and physical pain.
Is leukocytes are always a bit high and no one knows what kind of problem she has.
Is rheumatism test and C protein reactive is negative.
If someone hugs her, she feel pain. The pain improves with SSRI.
Recently, she tells me about a "ball" near the elbow. It's a bit protruding and moves if touched (I don't know how describe in English).
I want to help her so I ask you if anyone has an idea of this problem.

Luis, I think a SAMe trial , starting with the minimum dose, is well warranted.

Should symptoms improve, then the dose can be increased gradually.

This might help with her allergies in general.

Regarding thyroid fuction, due to biological and external stress, should she have low adrenal function, not only cortisol gets dysregulated, but also
thyroid function is lowered, for adrenal recovery (secondary hypothyroid).
This does not show in standard serum thyroid testing TSH, T3 and T4.
It would appear on Reverse T3, but she would also need Free T3 and Free T4 levels.
Secondary hypothyroidism does not rule out primary hypothyroidism type 2,
which is missed by the standard tests, as well. In type 2 the serum levels are normal, however, thyroid function in the tissues is low.
It's similar to diabetes type 2.
Again, I'm not familiar with the medical system in Italy, so I wouldn't know if
Free T3, Free T4 and Reverse T3 testing is available there.

Seronegative is when the antibodies are not present in the lab tests,
and yet the patient has all the clinical characteristics of the suspected condition.

There's speculation that seronegativity is a sign of less severe disease and perhaps slower progression, but again, there are many exceptions,
as no 2 patients are the same.

I forgot to say she has also nichel and herbs allergy.

How do you understand I'm Italian? :)

Is possibile to be seronegative and have inflammation problem?
What do you mean when you say seronegative? Negative C-protein and Reuma test?

Her thyroid exams are ok.
In Italy is difficult to find a Naturopathic doctor who test for adrenals... in Italy there isn't even the salivary test :(
I also think she has low cortisol...

Thank you, now I'll tell about methylation so she can test it.
Is safe to take SAMe to test or it has side effects?

My suspicion is that:

1.She may be seronegative ( any underlying infectious condition could be inactive, present only in the tissues, or other unknown reason).

2. She may have a MTHFR gene mutation, which could explain her D. Urticaria at least partially. When this mutation is present, methylation processes are impaired and Histamine (which is in high levels in D. Urticaria) does not break down efficiently, thus accumulating in the body.
She can get tested for low levels of SAMe or methylfolate, because in MTHFR gene mutation  methyfolate which is needed for SAMe production is deficient (methylfolate does not get converted  from its inactive form folic acid without MTHFR enzyme). SAMe helps break down Histamine.
She can also go for MTHFR gene screening or testing, if this is available there.
Note: Her Methylocobalmin B12 levels would be low too , even if her B12 levels show normal. She needs a separate test -Homocysteine Cerobrospinal Fluid testing for MB12 (neurological B12)

3. Low thyroxine (T4 thyroid hormone) could also cause similar effects,
specially if she's hypothyroid.
T4 is needed for converting B2 to its active form (I forget the name) and without this active form it lowers the effectiveness of the MTHFR enzyme dramatically!
So it's just the same as having the MTHFR gene mutation/ polymorphism.

4. Because of ongoing biological stress she may need to get tested for low
adrenal and low thyroid function, but it's best if she goes to a Naturopathic Doctor.
I find that conventional Doctors are not very the best necessarily, to diagnose and treat these conditions.

Thank you very much for the help.
She went to many doctors for the diagnosis but no one wants to help her.
She suffers many years of pain and dermatographic urticaria.

Does seronegative means that exams (reuma test and c protein reactive) are negative and she still could have arthritis?
I tell her about bursitis hoping that she won't think about cancer (she have also anxiety).
I know that this is not a diagnosis but no one can say what she have.

Hi Luis and welcome to the forum.

The Olecranon is the prominent bone at the back of the elbow.
Bursa is a small sac with liquid which helps with making movement  between bones that stick out, smooth and "fluid".

Any injury from repetitive use or excessive strain or friction, cause inflammation and swelling, with more liquid building up in the bursa.

This condition is called Olecranon Bursitis and it may resolve on its own.
If not, the doctor can treat the inflammation and reduce the swelling, but
care must be exercised not to re-injure.

It could also be inflamed because of undiagnosed arthritis (like seronegative
arthritis), or it could be idiopathic, meaning no known reason.
