Dear Pebbles-
I am with Selma on hoping that the problem is merely PND however, I had a similar problem with choking and gagging in my sleep and it was due to my lower esophageal sphincter failing as a result of a large hiatal hernia. This became a very big problem because I ended up with aspirative pnemonia and in the hospital for 3 weeks (12 days in ICU) most of this would have been lessened but we don't know how long I was deficinent of oxygen as I am the last to leave the house in the morning and I wasn't found until late in the evening- this of course is not an issue that a 3 year old has. I don't want to scare you but I would consider pressing the PCP for a referral to a gastric specialist just to make sure that this is not the case would be worth the time and energy.
Consider your tiny one prayed for.

Hi Pebbles and welcome to the Gerd forum.
I am so sorry ur little one has this issue and u as a parent seem so stressed.
May I ask did u try elevating the head of ur DD bed?.....also was she checked for post nasal drip. PND will cause the gaging and the vomiting and acid reflux.
It sounds to me more allergy related at this time...and that is as another mom, I am not a medical proffessional.
I pray ur DD will only need a saline nasal spray to help with the phlegm.
I am not sure how they would do the testing on a child this age.....but do ask the dr if it is a possibility or if it could be PND.
Good luck at the drs and please keep us posted.
