Chronic appendicitis exits?

A long story short. I have been suffering from chest pains for 2 years. Multiple visits to cardio no specific diagnosis other than GERD. Taking PPI for 2 years. Six months back I got pain at Right Lower Abdomen (RIF). The local GP thought Sub acute appendicitis and prescribed antibiotics. My WBC at that time was normal. The pain was intermittent, some times sharp and mild. I never took the antibiotics. The next day the local GP said no appendicitis. After 15 days still the symptoms continued occasionally and a went to a Gastroenterologist who advised to take Taxim-O an antibiotic. I took it for 5 days. I took an ultrasound of the abdomen and the doctor said no appendicitis. But I pestered him with questions about my symptoms. He then took a screening CT in the lab and gave the report as Chronic appendicitis with 7mm thickness. Ficolith inside the appendicitis and advised to remove it whenever you feel free but not an emergency. The Gastroenterologist was not convinced and he asked me to take a Contrast enhanced CT. But the local doctor said it is not necessary and i don't have any acute episode and there is no such Chronic appendicitis. Months passed and now almost six months with symptoms on and off at right lower abdomen as described above. I can feel discomfort while wearing tight pants and belts moving legs in awkward positions etc( but not always). I went to a new Gastroenterologist who i visited for 3 times was not convinced with appendicitis. But as my symtomps continued (mildly) he advised Contrast enhanced CT which the previous gastro advised. The local doctor says now that I can follow what the new Gastro says as they both are colleagues. He gave an order for the CT with query for appendicitis or Ileocecal tuberculosis. The same local doctor said no appendicitis or mass now recommends CECT because his friend that is the new gastro advised for it. I don't have diarrohea, fever or vomitting, blood in stools but a little weight loss over a period of year as I avoided junk foods, eggs, icecreams and potato after I got chest pain problems assuming it might be against heart health. So reduction in 4 kg of weight over a period of 2 years is a possibility. I want to know whether a mass could have been detected in the previous CT screening and if so why the report was like chronic appendicitis? What will be symptoms for ileocecal pathology.

Hi, appendicitis as such is more of a clinical diagnosis. In chronic appendicitis what we see may just be thickening of the bowel wall or appendix on a CT scan. But usually the image findings are correlated with clinical symptoms and blood tests. If an acute infection the neutrophils are raised and if due to tuberculosis, the lymphocytes are raised in blood counts. Since you don't have any clinical symptoms other than the occasional discomfort, you can wait and watch and based on the recent blood counts and the CECT findings your doctor will decide on the further course of action. Thank you madam. The GE now says the fecalith might have come out by this time out of the appendix due to peristalsis. Could it be true?
