7wk, 5 days, no heartbeat

I am 7 wks, 5 days pregnant and just had my ultrasound.  My physician wasn't able to detect a heart beat, the the sac was visible.  An HCG test the same day indicated that I am at 5700.  During week 4 my numbers were at 90, then 270 during the first few tests.  My cycle length is anywhere between 38 days-60 days, although I conceived using femera.  

I go back in a week for a follow-up ultrasound.

My question is, could this be a miscarriage?  I miscarried during my first pregnancy, but at 4.5 weeks.  This is a bit different and I'm not sure what to expect and am quite scared of going through this again.

I am 7 weeks pregnant with twins.  I had bleeding but the DR said @ ER my cervic was closed.  They did a ultrasound found 2 sacs in my uterus.  Now my DR said it would be a possiblity of miscarriage.  There is no heartbeat and they said my HCG level did not go up higher.  I still be the symtoms, my breast still sore, my nipples still tender.  I need help what to do???

Are you maybe not as far along as you thought you were?
