Cyst? Hernia? Ectopic Pregnancy?

Hello -

Last night I found a large protrusion from my lower left abdomen, almost groin, the size of a golf ball.  I immediately thought I had a hernia, as I battled a very nasty virus earlier this week and was somewhat of an olympic puker.  I went to the doctor this morning, who, after feeling around (as the protrusion had subsided quite a bit), found the tender spot and informed me it was my ovary (why, at 28yrs old I think my ovaries are by my hips, I don't know), and so she did a CBC to check for an infection, and I was fine.  She wanted to schedule a pelvic and vaginal sonogram for later this afternoon, but I didn't have time, so it's on the booked for first thing Monday morning.

I'm not in any major pain, the protrusion just has a sort of a dull throb to it and tender to touch and press.

After getting dressed and chatting with her, she asked the obvious:  is it possible to be pregnant?  I said no, as I'm recently single, and mentioned my period WAS late by a few days last month, but I was traveling in Peru and assumed it was from adrenaline.  Then, thinking about it, I'm mentioned that I am actually about two weeks late now.  I told her no possibility of pregnancy, but looking at the timeline, i DID have intercourse on 8/20 before my break.  Since I DID get my period last month, just a few days late, but haven't gotten it this month, is it possible I am pregnant and having some sort of ectopic complication?  Would that explain this golf ball like goider on my abdomen, or could there be a real problem here?

OK I understand now. Just wasn't sure if it was just the cysts or both.  I thought from the start the protrusion had to be a hernia, just from my own experiences.  

Yes the cyst can be the cause of your missed or late periods.  The cysts if left untreated can lead to diabetes or insulin resistance, heart disease, cancer, high blood pressure,  acne, excess hair growth on face or body,  hair thinning, mid section weight problems, and infertility.    

If the cysts haven't gone away in the 6 weeks they will start treatment or remove them.  It is possible that they might remove them when they fix your hernia.  The surgeon will be in there looking around a little anyway, but it depends on exactly where the hernia is, the size of incision, and the other visible areas he can see from where he is located whether he can or not.  

I have my hernia surgery on Dec. 6th.  My sis is having open incision pancreatic bile duct repair on Nov 5th and I decided to wait in case she needs my help after her surgery because they say her's is going to be a rough one to recoup from.  

My hernia surgery will be surgery #32 for me and I'm only 46.  So I'm hoping it will be the last one for awhile.  LOL I'm thinking there can't be too much left to go wrong inside of there which is what I also thought last year when I had a 3cmx3cm tumor removed from my breast.  

Anyway, hopefully these surgeries will get all of us back up and functioning normally. Yes, I have 4 cysts in addition to an inguinal hernia which will be fixed in a couple of weeks.  I've opted to do the open surgery over the laproscopic, as it seems it is very important in women to make sure that the hernia is repaired without any doubt that it will stay intact for potential pregnancies in the future.  Apparently only 2% of women get hernias, so here we are represetning :)

Could the cysts be the reason I haven't gotten my period?  I am not pregnant. Also, I've PCOS and don't have any of the symptoms other than multiple cysts.  If they haven't gone away in 6 weeks, what does that mean?

Hello, It is not uncommon, 4 is a lot though and the size is a usual size.   Cysts can be very painful. Most do not require any treatment.  Some do rupture at times.   The most common cysts are caused when the body forms a cyst like structure called a follicle in the ovary during ovulation which is a normal process, but when the egg is released this follicle ruptures forming a corpus luteum that will dissolve if pregnancy doesn't take place.  A functional ovarian cyst forms when the process doesn't take place just the way it should. Also, there is another condition called polycystic ovarian disease which is less common that can occur as a result of hormonal imbalances of estrogen and progesterone.   Hopefully when you go back in 6 weeks the cysts will all be gone.  

Do you have the hernia too?  It sounded like you do not.  But if so I can't answer the question until after 12-6 about the surgery but have been told by a friend who had it and the doc that it is pretty painful because of the manipulation of the intestines being shoved back through the opening and then the mesh being placed to hold it there.  LOL I'm looking forward to this YIPPY can't hardly wait.

Actually, I spoke too soon:  more test results this AM showed 2 cysts on each ovary, approx 1.5 something (mm?  cm?) in size.  The doctor says they are on the larger side of small and we will recheck them in 6 weeks to see whether they are changing/growing/etc.

Is it common to have 4 cysts and is that size about normal?  Any advice on what to expect?

Verdict:  hernia!  Located directly on top of my left ovary, hence the difficulty deciphering which was causing the swelling.  Ultrasounds verified the cause this morning.

On that note, what should I expect with hernia surgery?

Hello, Kate I too agree that it sounds like a hernia.  I have had all 3 the hernia which I'm having surgery for Dec 6th, the ovarian cysts, and an ectopic pregnancy-both of which I had surgery for years ago.  

The ovarian cyst would not be protruding to the outside there has to be some sort of weakness in the lining, or the area for that to happen generally seen with a hernia, and your puking episodes could have caused enough straining to cause it.    The ovaries are deeper inside, the cysts do cause a lot of pain, but do not protrude to the outside.  

If you do happen to be pregnant and got that way almost a month ago had it been ectopic you SHOULD have been having symptoms before now, but of course you definitely need to rule it out.  In order for it to have gotten that size without rupturing the tube would have been practically impossible.    With an ectopic pregnancy it is in the tube and as the pregnancy progresses and the egg grows the tube does not stretch causing pain and rupture if left untreated.  Mine showed no visible signs externally.  

The only other things I think it could possibly be is a tumor-but if it went down I highly doubt this being the cause or a muscle spasm-which is possible from the puking and muscle straining.  

A hernia doesn't go down but will appear to if you change positions.  Say if you find it standing up you may have trouble finding it lying down.  I Hope This Helps, Good Luck.  I'm heading out of town Sun morn and won't be back until Fri but keep us posted please and I'll check on your post when I return.

I think I agree...I imagine if it were anything else, I'd be doubled over and have many more symptoms other than the protrusion...I'll know for sure on Monday after the sonogram.  Thanks.
