Condom was on under the head of penis.

I had sex with a trans woman. We used condoms for penetration. I had unprotected oral sex but we both take meds for HSV. My big concern is would there be a risk for hiv? The condom was still on around my penis tip. I seen it with my own eyes. The person penetrating abruptly got off me and that could have been when the condom started to come up.

Sorry, we had anal intercourse. While pulling out. I noticed the condom had slipped up to the head of my penis. It was on and the head was the only portion covered. So I’m just wonder if the shaft was a cause for worrying or concern.

No. HIV enters through the urethra. You're fine.

I'm a little bit confused by your description of events, but as long as the head of your penis was covered by the condom during intercourse, you were protected from HIV.
