At no time did you have an exposure in any of the situations you've provided and you do not need to test nor is it advised.
I had the same questions myself when I took the home testing kits, I had doubts of their accuracy. After researching it, and actually calling an HIV/AIDS hotline for my state that I live in, I received plenty of reassurance that they are indeed very accurate, and are the same kits used in clinics to test people. So yes, they are reliable.

Ok. First things first. You don't know that the sex worker even is HIV positive. You can't assume that, but take necessary precautions just in case.
Anyways, you used a condom. I don't believe the bottom of your shaft being exposed to fluid would be a mode of transmission, unless you had a sore there, which you stated you didn't.
Secondly, you need to post this in the HIV forum. And take some time to read through some of the posts. There are MANY MANY MANY topics on oral sex, and you will see that in regards to HIV, basically there is no risk for oral sex. Giving or receiving, ESPECIALLY receiving. Saliva is not a good environment for HIV, hence why it isn't transmitted through kissing. The possibility of her "picking" up the virus that she left on your penis, and then having it in her mouth and it getting into your penis, is non existent.
Try and relax! Honestly, you didn't have a risk for HIV. However, if for peace of mind, you can get tested at the 3 month post exposure mark for a conclusive negative result. They now sell at home HIV testing kits at drug stores, easy to use and results in 20 minutes. But you need to wait at least 3 months after exposure to get a definite result. Having said all of this, you DON'T even need to test, but if it would give you peace of mind, by all means do so. Take care.
