Semen on Penis

Dear Doctors,

I need your advise for an risk assessment. I am a trauma surgeon from germany so I sometimes do have contact with hiv positive patients. But this is not my concern. After an emergency shift I made a mistake.

I joined a sex party and was engaged in a threesome. No penetration happened. A man masturbated himself and a woman. It was in a very dark room. I kissed the woman when suddenly the man touched my circumcised penis and gave it three strokes with a soaking wet hand. Mainly on the penis shaft but also slightly at the glans. I had a modified circumsion so part of inner foreskin is still present and was exposed also to the fluid.  I can´t tell whether it was vagnial fluid or his sperm. I stopped the threesome immediately dried my penis with a towel and left the room.

Presuming it was sperm and the male part of the couple would be hiv positive do I have had a risk contact and need an HIV test?
Other sexual transmited diseases won´t be a problem because I took doxycyclin.

I could perform a Roche Cobas Taqman 2.0 HIV 1 RNA PCR at day 10 after the event in a lab near our hospital.
As far as I read on your page, and this would be my second question, at day 10 it would have a reability of 99% ?

Not a risk , HIV is never transmitted outside the body, the air would kill it.

But if you cannot put this behind you , just get a 4rth generation test in 28 days, only to see a negative result, i have read around that they are more reliable than a PCR on early days.

It´s good to read reassurance.

In germany you typically learn I´t can be risk. Also if you read on german hiv goverment pages they call it a risk. A small one but yet a risk.

Meanwhile I read a lot of articles here. MD Handsfield and Hook write the same as you. I think I decide to believe them and your opininon and will move on. Thanxs a lot.

So you are a surgeon and don't know how HIV is transmitted?
