Treating A-fib

I am a 33yr.old female with a-fib. I am taking amiodarone, cartia, coumadin, aldactone, lasix, midodrine.  I am NYHA stage2.I have a pacemaker for S.S.S. My a-fib generally is better controlled on these meds.  but in the last month my rate feels very irregular, sometimes for several hours a day, and other times several minutes on and off all day.
Question:
How do I tell the difference between a-fib and this being simply lots of PVS and PACS? It skips and beats fast irregularly and I don't feel well.
My second question relates to a cardiac cath. This is very confusing to me. My C.O. was 16/l/min, which you and my cardio said was high.  However, when I asked if the high CO was causing my heart failure symptoms, he said no, it was because I have a young, strong heart.
Question:
How can this be, with stage 2 heart failure, SSS, and a-fib?  Do you have any thoughts about this?

One more thing. My liver enzymes are going up steadily.  They are now about 3-4x normal. I have terrible itching and pain on my right side.  They said they really want to keep me on the amiodarone even though that is the cause.  They have tried every other med.
Question:
Do you think that sounds risky for my liver?

Thankyou so very much.  Have a great day.

Dear cn,

The only way to tell for sure is with an ECG tracing at the time of the irregular beats but I would suspect that these are afib episodes.  As far as the cardiac output (CO) I would think this would be a very likely cause for the heart failure.  If it is not  your cardiologist must find another explaination for the heart failure.  The liver enzyme elevation could be a symptom of heart failure, due to the amiodarone or due to something else alltogether.  I would be very cautious about continuing the amiodarone in these circumstances.  I don't remember if you have had a second opinion at a tertiary referal center (large medical center) or not but if you have not I would highly recommend doing this.
