Arrhythmias

I am at a loss now. I have had nsvt, pvc, pacs for 8 yrs now. Two attempts at a ablation no luck. Now I have devoloped svt. My cardio wants me to come check into hospital to start flecainade. Im not sure about this. She also said a ablation would not work due to it being multi focual..I would rather try another ep. The meds scare me to death. But how can one person be so lucky and get both of those arrthymias? I am at a loss here please help. wmac

During my EP study I was not given Versed at all.  My EP wanted to be able to induce my problem.  I was given something else that wasn't as strong.  It did hurt a little, but nothing I couldn't stand.  

What did this EP say?  What did he do for you?  Are you back on medications?  I would ask to go back on a monitor.  Surely over the period of a month something should be caught if you are having all these problems.

Well had eps on thursday. It didnt go to well from the moment I walked into the hospital doors. Anyway once again they could not induce my nsvt or my svt. I dont get it. He couldnt even get one single palpitation. But yet after the eps in my room my heart was having all sorts of pvcs. I am so discouraged. The cardio tried some stuff that was equifalant to 8 cups of coffee got heart rate up to 160 with nothing  then he used isoproterenol and got my heart rate up 300 beats per min. still nothing. but with it being that fast i dont know how he would see anything abnormal or not. But the point remains im still not fixed. My heart has been acting up of course ever since. I cried all day in the hospital. I am at the end now to where I have to admitt I have to live with it. which in fact i have for eight yrs but now for some reason its really hard to deal with. Does anyone have any suggestions? Any would be greatly appreciated.

So are you saying that its not worth it for me to try another eps/ablation? I had two before with no luck.. They could not induce it. Also he told me that with having two before with no luck the chance is low but its just the way it goes sometimes and may this time maybe we will get lucky. He is not the one who did it before. He did seem a bit more confident about getting the svt induced. But said he would also try for the nsvt as well.
This is pretty much my choice. He said he wanted to help me because he knows how much this controls my life. I live in a small town and my biggest fear is that my arrthymia will start up and not stop and I will have to go to the er here and that one freaks me out... Any advice from anyone would be helpful.

The month before I had my ablation, I had a ton of testing - no one ever suspected from those tests that I would have malignant pvc's or VT; they just said I had a TON of pvc's that were damaging my heart.  I was told I had valve damage and that was possibly the problem; which I did have some valve dmg just not severe. (that I know of)

During the procedure, my EP mapped a place at the RVOT that I went into sustained VT and polymorphic in nature that caused my heart to stop.  He ablated the area and found 2 more areas that were the same near the same place and stopped my heart twice more... he ablated the areas.  He thought it was due to ARVD, had me go through an MRI which was inconclusive; but I still have no clue what caused mine.

Then I had a cardiac cath to check for kinks, clots, plaque etc and I came out squeaky clean, albeit a dx of EF 40% and confirmation of non ischemic cardiomyopathy.  I had a pacemaker/icd implanted due to my bradycardia/vt episodes.

If you're very symptomatic, I would try the ablation (cryo?) and see if it helps.  I'm looking into dr's for that now so when I go back in Nov I can get it done and hopefully alleviate some more stress on my heart from increasing problems in the past 6 months.

Cindy, This is a different ep cardio. I saw this one three yrs ago who is very experienced and that is all he does. I had gone to the university of utah before with two different docs and neither could induce my arrthymias. Doctor weiss does very complex ablations he said if they couldnt induce the other two times he said its doubtful he could. But he said he would try. My nsvt or svt are not life threating but very dehlbilitating. (cant spell) he just doesnt want me to be disappointed. which i would be if they dont get it.
Lisa my way of thinking is like yours its worth the risk to get rid of this terrible thing that has controlled me for eight yrs now. What arrthymias did you two have? I have also have several opinons probably too many they all say not life threating live with it but thats really hard to do. I want them gone for ever..

What about going to another doctor at a larger medical clinic or university? Some have had great success going out of the city/area they're in and getting another opinion.  I thought I would stop at 2 opinions, but I didn't I went to 3 to see if that one said anything different, but my case was a bit different.

It's normal to be anxious and have a bad feeling.  I had no clue and didn't talk or think twice about anything happening during surgery, I had other types of surgery before and never had any problems, but with any surgery there are risks.

My way of thinking was it was worth the risk to rid myself of these dreaded things that run amok in my chest making me miserable =) daily.  Hopefully you can find the right answer and a doctor that will make you feel comfortable with them getting to the bottom of your problems.

I think we all got that "bad feeling" before our ablations and thats why we wrote our kids letters, made all kinds of deals with God, the whole nine yards.  Its scary when anything having to do with the heart is undertaken wmac.  Are you saying that you are going to go back to the original EP that did the other ablation or mapping?  My thoughts are if he/she couldn't generate the electrical impulses the first time what makes them think that they could do it a second time.  I wasn't clear on that one.  I had my ablation done a couple of years ago almost four years now actually for some pretty heavy duty SVT's and it was 100% successful for them and they are now a memory for me.  I chose Versed as well and my understanding of the med is that it puts our "minds" in a twilight state not our hearts so correct me someone if i am wrong on that one.  Its an amnesia med so that we have no memory of the procedure.  We aren't asleep with Versed that would be general anesthesia....personally if you have the right doc i would go for it....i would seriously question any doc that said to me that because of my prior hx. he may not be able to induce either....ablations are not for the faint of heart docs that are already making an excuse before you even get to the lab....you need a doc in my opinion that is tried and true with over 1500 ablations under his belt w. no incidences of hitting the AV/Sinus node and no losses...i chose a world known doc that knew how to color outside of the lines so to speak and neve gave up until he got it....i owe him my life...as far as your daughter goes wmac...i have two and i think they were already dividing all of my things up as i was wheeled in because they had such a bad feeling...our daughters are just scared because they love us and don't want to lose us...but you have to reassure her that we are not the navigators in life only the passengers....i know how you feel tho...i hope this was a little helpful......

Went to ep/cardio in utah yesterday.. He said that he would recommended flecinaide as well its only dangerous in people with heart disease.. But what if I take it and and develop it down the road then im sol as well. I just cant do it. He agreed to another eps and possible ablation.. Which is what I wanted but i just cant get too excited about for some reason.. He believes the svt is one foci as the other doctor said multi foci. He is just concerned beings that I had two eps before with the nsvt being non inducable that he may not be able to induce it either. He said this time he would do it different than the two other docs did it. He would keep me totally awake  put iv in and give me some drugs and caffine through iv with hopes my heart would go into arrthymia then put me out. He says if i had too much versede my heart may have been too much asleep to go into nsvt. Im a bit nervous this time and my daughter is begging me not to do the eps ablation she says she has a bad feeling.. Any advise from anyone..

I took the  first dose last year and within the first hour, it made my HR & BP keep dropping, chest pain and tachycardia so I called the dr who prescribed it  and they sent me to the ER; even after 11 hours they couldn't get my stats above hr 41 & bp 70/40 so I had to wait it out and let it run out of my system

I don't blame you;  I would get a 2nd or even 3rd opinion just to be on the safe side - while Flecanide has great results, I'd be leery of the side effects it can have since you've had problems with Toprol

I shudder when I think of the side effects I had with my first dose of Toprol and ended up in the ER; but I did tolerate Zebeta and Inderal a bit better, so hopefully there's a med for you =)

Well I canceled my hospital appt. it was for monday to be admitted to get started on flecanide. I scheduled another appt. with another cardio in utah. I have seen him once before. I do really like him and trust him. I am going to ask him about another ep study and ask him his thoughts on flecanide.
I am not currently on any meds as of now. I tried toporal before and the third day into it I had the worse nsvt run ever.. It got recorded so I know I had one along one. So I go to utah on the 7th of sept.

Many people get effective results with Flecanide but it hast to be closely monitored in the beginning to be sure one doesn't have severe side effects... I understand, I have never taken it.

What meds are you now taking, any?

If you are younger, say under 50, I think a cure should be sought, and think that is not Flecanide.  Perhaps another EP can deal with the complications of you ablation status, or at least can confirm the diagnosis.  Of course, if you trust you doctor, it may be hard to change... change as a second opinion search so you can come back if you decide that is best.
