I assume there's a connection between the ECG and the planned neurosurgery. That being the case the doctor should advise you if there is any neuro concerns. As for heart concerns, that's best asked from a cardiologist.

I think it would be unusual for PVCs to stand in the way of your surgery. Your neuro doctor will get a clearance from a cardiologist. While on the operating table you will probably be fitted with an external defibrillator in case you develop an arrhythmia.

That is correct, an arrhythmia is when the heart beat is irregular. In otherwords, the regular heartbeat is interrupted. A regular heartbeat originates in the sinus node. A PVC originates in some other place in the heart and interrupts the regular beat.
