Hbv DNA

Sir My HBV DNA log value is 8.04 log and sgpt is 61 and also still HbeAg positive.
So what can i do now??
And i also want to know that is 8.04 log value is Danger ??

You should test your kidneys, and if they are OK, then your best choice, because of your high DNA level, would probably be to take either:

a) Tenofovir (but if your kidneys are not 100% then take Entecavir instead)
b) PEG-Interferon
c) Both Tenofovir (or Entecavir if kidneys aren't 100%) and PEG-Interferon at the same time.

If you can find a good hospital and a good doctor then maybe the option c) would be the best choice for you. You will need to be monitored frequently when you start your treatment so as to make sure no side-efects develop.

Change your doctor IMMEDIATELY !

Sebivo = Telbivudine, which has very high chance to make your hepatitis B to mutate so that your body won't be able to fight against it efficiently anymore, and also make other medicines inefficient.

Your doctor's licence should be taken away, that person should not be practicing medicine.
You are likely to find more knowledgable doctors in big hospitals that have liver/gastro/hepatology departments. Do you you live in a larger or a smaller city, and what country do you live in?

Only 3 medicines are good for hepatitis B: PEG-Interferon, Tenofovir, Entecavir.

Thnx For your Reaply.
Now I am under treatmeant by a Hepatology specialist Doctor.He advise me Tab. Sebivo (600 mg) once per day.Is that ok sir and also want to know that there is any chance in fututre to be my HbsAg negatibe?

Your fibroscan is on the edge between normal and slightly elevated, which could be also the result of the current inflamation of your liver as your ALT is 61. There is not significant fibrosis which is excellent.

However, on your place I would follow the British/NICE guidelines for treatment of Hepatitis B:

and start with the treatment unless your ALT and DNA start rapidly improving in 3 months. So you can do one more measurement 3-4 months after your last measurement, and if ALT and DNA aren't significantly improving, follow the British guidelines:

"Offer antiviral treatment to adults who have HBV DNA greater than 20,000 IU/ml and abnormal ALT (greater than or equal to 30 IU/L in males and greater than or equal to 19 IU/L in females) on 2 consecutive tests conducted 3 months apart regardless of age or the extent of liver disease"

As you are young, if you can obtain both medicines at the same time, you might want to consider starting with the combo therapy of NUC (Entecavir or Tenofovir) + Interferon. Try to find a good hepatologist to take you trough the therapy as interferon can sometimes have side-effects.

How old are you?

You need to do both 1) Fibroscan and 2) ultrasound of your liver.
