Hepatitis B viral load

I found out that I had Hep B when I was 19. After research we have realized I have contracted it at birth. I've been on 4 trials of Alpha interferon and failed all 4 trials. 10 years ago my viral load was 10mil. Last years it was up to 176mil.  All my labs are normal. The only thing that is elevated is my ALK and Bilirubin. They said that I am listed as chronic carrier state. They are not putting me on any treatment at all. Stated that it wasn't needed. Should I be concerned about my viral load increasing? What kind of damage can it do to my liver with it increasing. Should I be on treatment? Any advice is much appreciated. Thank you in advance.

My mother was diagnosed with cancer sometime in here 30's. I hope I answered your question correctly. If I misunderstood I apologize in advance.
Just need to clarify. She had cancer in her 30's but have been cancer free for many many years.

Sadeyesamc, maybe you can get a second opinion.  I went to two different doctors.  I'm also 39, female, just delivered my son a month ago and I have high VL 2 million and I'm thinking my ALT is high bc I was pregnant so I waited until after my son was born so I could be off all the other meds that I had to take after pregnancy.  I just had ultrasound done and waiting in results to see what state my liver is in -- very nervous.  I've had hbv since birth.  I think I will request for another blood work to see where my ALT is but doctor says that he can now prescribe enteclavir now that I'm not pregnant anymore.  Did your family on enteclavir have any side effects and how long have they been on the treatment?

Does any one else of your family members have chronic hbv. Sorry for asking this question.
Reason is that you should vaccinate all of your family members.

It's all good. All my children are vaccinated. At the age of 1 they all had their titer test done to make sure the vaccine worked. One of my children had to take the vaccine twice because it didn't work. Thankfully they all are negative.

FMHX:
My mother has Hep B- developed Liver cancer. She has been on Entecavir so now she is at undetectable levels.

My brother also took Entecavir and is also undetectable levels.  I think his viral load is now at 20. That's a rough guess.

My grandmother had it don't know to much about her.

My uncle- pass away from complications from Hep B.

My grandfather also passed away from complications from Hep B. Liver cancer.
Also, my husband contracted it from me but his body fought it off. So he's now immune.

Difficult to answer your questions. At 19, you would most likely be in the Immune Tolerance(High replication, low inflammation phase). Then you had 4 trials of Interferon, 48 weeks per trial? When you say 10 years ago, your viral load was 10 million, at what age were you? Now your viral load is 176 million. The high viral loads are not unusual for someone in the Immune Tolerance phase. If your ALT has always been persistently normal and you are under 40, then treatment may not be needed. Anyhow, to be sure, I would do a Fibroscan to ascertain the state of your liver. If there is significant fibrosis, I would consider treatment with  TDF or ETV, after consulting with doctor.

I'm so sorry for not making my post clear. I had 4 trials but never totally completed the trials. I have been removed off of them early due to my white blood count dropping so low. Also, they removed me off of it because they noticed no change. My very first trial was back around 97 or 99 can't quite remember. My most recent visit doctor has stated that my body is just pretty much living with the disease that my body thinks it's suppose to be there.

In 97 my liver biopsy showed minimal damage. I am currently 39 years old. I consistently have some form of stomach issues that they can't figure out. I also consistently have pain under my rib cage.

I'm just a bit uneasy with the viral load increasing because of the FMHX with hep in the family. In all honestly I just want this out of me.
