hbv dna rising on Tenofovir from 1 year

Hi Guys,

i have been on tenofovir from 1 year 1 momth ...i get my blood work done regularly after 2-3 months I am pretty much dissapointed with my latest results:

Jan 2017 Hbv dna - uncountable >170000000 iu/ml
march 2017 hbv dna - 200000 iu/ml
may 2017 hbv dna - 9000 iu/ml
aug 2017 hbv dna - 1125 iu/ml
Nov 2017 hbv dna -425 iu/ml
feb 2018 hbv dna - 6500 iu/ml

fibroscan jan 2017 - 7.7 kpa
feb 2018 - 8.6 kpa

Alt/ast -69/40


Now doctor has prescribed me more tests
-hbv resistance and genotype
Doctor was saying he will add enteciver with tenofovir for 3 months and withdraw one medicine when virus will get undetectable or he is also considering TAF as an option.

Also I am hbeag +ve and age is 25 years

Please members help me out...

Hi guys, my doctor has switched me on taf....the resistance test showed nothing and genotype test showed nothing because of very low viral load...doctor said to me that ma fibroscan of 8.8 kpa can be because of cap score of 245 on fibroscan which indicates of mild fatty liver.
However, he strictly told me to avoid taking i ornithion and silymarin because of drug interaction and told me that my fibrosis will reverse.
I have been advise to take below medicines:
Tafnat 25mg generic version of Taf

vitamin e tablets brand evion 400 mg

multivitamin riconia lp

I am happy that i have been switched to TAF

Yes this is worrisome but i think a repeat test 2-4weeks later will make the picture more clear. The dr has no other options than these 3 meds. U should repeat the test and see. Don't Panic things will normalise and u will turn hbeag negative soon. Currently take medication of silymarin and l ornithine and l citrulline to improvise liver health

DON'T take silymarin unless guided by your liver specialist as it has intracellular effects and may interfere with Tenofivir eith TDF or TAF...

Thanks brother could you please give your opinion on this too  antivirals does not work in immune tolerence phase and how can i reverse fibrosis

In cases of very high starting levels of hbv dna, a partial resistance to tenofovir  is likely. The doctor is doing the right thing, adding entecavir for a while and then testing if it can be withdrawn.
hi studyforhope can you please tell me how to reverse fibrosis and in immune tolerence phase is it good to continue medicine also whole year my bilrubin kept fluctuating mostly indirect bilrubin
Rohan - In an ideal immune tolerant stage, your ALT would generally be around the normal values or < 2 x Normal Values. Normal being ~30 IU for men. Your ALT is slightly higher than what it should be. You are not in an ideal immune tolerant phase as your immune system is fighting the virus and hence the fibrosis and raised ALT. What the doctor is doing seems reasonable and hopefully you will seroconvert to HBeAb soon after Tenofovir or Tenofovir+Entecavir. Seroconversion usually happens in the 3nd decade of life (assuming you acquired the virus during early childhood). Fibrosis will start receding once your HBV DNA is undetectable and your ALTs normalize.

hi living with hep b today i got my ultrasound report that is fine also currently i am on tenofovir from past 1 year and doctor will put me on tenofovir +enteciver from next week.Hope i will seroconvert also i read an post by stef where he mentioned it is good somehow when alt increases and some viral load increase it means hbsag is clearing fastly and there will be some damage done which afterwards can be reversed?Is this a good immune response?

also i have it by birth as confirmed by mother’s test she also have it last year we got to know about my mother.when mine was detected
I seroconverted from hbeag+ to hbeab+ 10 years ago after 1 year on adefovir dipivoxil and 2 years on entecavir. I was 22 yrs old when I seroconverted. Luckily I have been inactive since then with very low HBV DNA and low HBsAg.

Thanks a lot brother for the information.However i feel tenofovir is a very strong medicine from past 1 year i have lost 9-10 kg weight on it and from last week i feel some problem in breathing ..Also doctor recommended me tests such as ttg iga, thyroid and blood sugar.All tests were perfectly fine. Tomorrow  I have appointment with my hepatologist. I will ask him to switch me on Taf + etv or enteciver. On last visit my doctor told me Do not worry you are very young just enjoy your life normally study whatever you do we have lots of options do not worry. However i am feeling bit stressed out. Even my friends and society is very good they know about my disease and support me alot
