Find a Fibroscan which has ability to measure CAP parameter and measure your fibrosis as well as steatosis.

You might need to start with entecavir/tenofovir...

Well, you have provided scant information. Your viral load (hbvdna) is high, typical of someone in the Immune Tolerance Phase. Your ALT is elevated, so it could mean you are into the Immune Clearance Phase. Not knowing your history, these are all just guesses. Ayurvedic is not known to cure HBV. I suggest regularly checking of your ALT, hbvdna, and HBeAg status. The road ahead should be normal provided you get treatment when it becomes necessary, it will help to keep your liver in a good shape until a cure comes along.

There are more details in my anoher thread, request you to comment on it please .
Personally, I think you(or your brother) should start treatment, as his hbvdna and ALT have been persistently high for several years now. It is most likely he is now in the Immune Escape phase with active HBeAg negative chronic Hepatitis B.
I don't mean this to be a criticism - it is better for you to start one thread and give as much information as possible. We are not doctors here, we try first of all, not to do harm, but you are expecting us to give you opinions based on scant information, but sometimes followed by additional information that may cause us to change our original opinions.
