another study confirming 60% hbsag clearance by peg add-on

Abstract
Background and objective
The aim of this study was to prospectively evaluate whether the addition of peg-IFN to a stable NA regimen leads to loss of HBsAg in HBeAg-negative patients with chronic hepatitis and HBV DNA fully suppressed by long-term NA treatment.

Study design
We analyzed HBsAg levels in 10 HBsAg-positive, HBeAg-negative patients who received peg-IFN alpha-2a in addition to a NA regimen. Treatment lasted a maximum of 96 weeks, according to changes in the HBsAg titer. Before peg-IFN therapy, HBV DNA levels had been below the limit of detection for at least three years.

Results
HBsAg levels declined in nine patients. Among these nine, four became HBsAg-negative after 48 weeks of peg-IFN treatment; these patients received peg-IFN for only 48 weeks. NAs were stopped in these four patients, and these levels remained stable for at least 18 months (loss of HBsAg; HBV-DNA negative). HBs seroconversion was observed in two patients. The remaining five patients received 96 weeks of peg-IFN therapy. One patient became HBsAg-negative at the end of peg-IFN therapy; another became HBsAg-negative six months later. Three patients did not become HBsAg-negative. NAs were stopped in the two patients who became HBsAg-negative with no relapse during 12 months of follow up.

Conclusions
In HBsAg-positive, HBeAg-negative patients with HBV DNA were fully suppressed by long-term NA treatment, the addition of peg-INF for a maximum of 96 weeks based on HBsAg-titer monitoring led to a loss of HBsAg and cessation of NA therapy in six out of ten patients, with no relapse for 12–18 months of follow up. HBs seroconversion was observed in two patients.

For anyone looking about any update from Replicor ,,, I just emailed them couple of days ago this was the replay :

We are planning a new trial in western Europe which should hopefully start very soon. There is no trial yet planned in the USA.

I would expect that you will hear more news from our website over the next 6 months about our new results.

Things are progressing well although always too slowly for those waiting for the treatment to become available.

There are ongoing studies for TDF+IFN combo but not sequentially only 2 months together then only IFN for 48w or only TDF for a year or two cant remember. Results 2015.

This is quite an important  study.  Thanks steff.  I often have some doubts about this but this study helps support what you have been saying all along.  However, this is a rather small study.

mixed genotypes,

sequential treatment reactivates our immune system and pegintf works totally different indipendent of genotypes, mutants

it doesn't mention the genotype.
I have genotype c which my doctor told me has a very low chance of working with interferon.

alt is normal with pegintf add on, the clearance is usually by purgining cccdna from infected cells, on this trial alt was normal before peg add on and after

age is not meaningful to this protocol

it is welknown that hbsag is lower with age without any therapy but this has nothing to do with the research.response to pegintf is indipendent to hbsag level and age, it is only correlated to antivirals like tdf or etv rescuing immune system cd8 to response to peg

The mean age of this group is 58 (41 - 76), it seems that qHbsAg is gradually declining with age increased as well.

In the four patients who remainedHBsAg-positive, HBsAg levels were lower at the end of the follow-up period than before the addition of peg-IFN therapy: 147 IU/ml[95%CI 0–580] vs. 1233 IU/ml [905–1561] (Fig. 2).

median 147iu/ml is a very good result, they might even clear hbsag slowly in the follow up 5-10years

dont expect anything from myrcludex, the dose used was too low

was the russian trial pegintf plus tdf or etv plus myrcludex?

I wonder why myrcludex is silent, the trial must be already over!
