Changing Telaprevir dosage from q8h to twice daily

Hi all.

Now i'm on third day of thriple therapy with incivek.

It's 09:26 am now.

First day: I've shot first injection at 6:00 pm + 3 ribavirins at 8:00 pm and 2 telaprevirs at 00:00 that night.
Second day: 08:00am 2 telaprevirs and 3 ribavirins + 04:00 pm 2 telaprevirs + 8:00 pm 3 ribavirins + 00:00 2 telaprevirs.
And this is third day morning and i've got 3 telaprevirs and 3 ribavirins at 08:00 am

I changed my dosage from q8h to twice daily . I will get drugs only at 8:00 am and at 8:00 pm as 3+3 (with 20 gr fat) . It's fda recommendation and comfortable for patients and no difference on SVR rates. http://www.fda.gov/ForConsumers/ByAudience/ForPatientAdvocates/ucm372817.htm

I've forgotten this and remembered now and changed dosage.

Anybody knows is it true to change telaprevir dosage after start? IS there anything about resistance? And is it problem to get them with ribavirins at same time with fat??

If I were you, I would check with my doctor to be sure it is okay to change the Telepravir dosing from every 8 hours to every 12 hours. The studies show that the success rate/cure rate (SVR rate) is about the same if a person does the every 12 hour dosing or if a person does the every 8 hour dosing.

Take all of your Telaprevir doses. Telaprevir does not have a long half life and the blood level will start to drop after a few hours, so it is important to take every dose. However, the studies do show that taking it every 12 hours is as good as taking it every 8 hours. So taking it at 8 am and at 8 pm should be okay.

Ribavirin does have a long half life and it will build up in your system. But you need that high level in your blood stream so it is also best to take all doses of Ribavirin.

We all have resistant strains of virus even before we start treatment. With treatment, we need to prevent viral replication, kill the virus, and prevent the already present resistant strains from multiplying and taking over.  The reason we need to take the pills on time is to keep the medication level at a high enough level so that it will prevent the virus from replicating and to allow our immune system to kill the virus. The Telaprevir prevents replication of the virus. The Interferon along with the Ribavirin boost the immune system so that our own immune system can kill the virus. We need to take the medications on schedule and for a long enough period of time to accomplish this task (eradication of the virus).

The pills (Telaprevir and Ribavirin) can be taken at the same time with a fatty meal. It is important to eat 20 grams of fat with each dose of Telaprevir (with a meal) (for better absorption). It is also important to eat a meal with Ribavirin but the meal does not have to contain 20 grams of fat. Ribavirin does absorb better if taken with a meal.

I treated with Telaprevir, Interferon, and Ribavirin. I treated for a total of 48 weeks (12 weeks Telaprevir, Interferon, and Ribavirin, and then an additional 36 weeks of Interferon and Ribavirin). That was Sept. 2011 through Aug. 2012. I attained SVR (cure) and am now 2 years virus free. I feel so much healthier and I have no fatigue now. I am doing very well.

Thank you all. You made me feel better.

I've also asked doc by e-mail and he replied to continue twice dosing and im on right way. It's not about resistance.

Now i can sleep well. Thanks again. I will be here next days.

Hi, I'm currently on week 12 of this treatment in Canada, so I thought I'd chime in - I'm trying to understand your question though so if I understood it wrong let me know.

In terms of telaprevir dosage, I started treatment immediately with 3 telaprevir + 3 ribavirin every 12 hours, with 20g of fat. I pretty much follow the same schedule with you. I don't think you'll have a problem with changing your dosages, as the study showed there's no difference between the two routines anyway. From my understanding the point of these drugs and the long therapy time is that you accumulate them overtime anyway (correct me anyone if I'm wrong). So just keep it up and don't miss any doses.

I don't think you need to worry about resistance at this point. Resistance really is due to the mutation of the strain of HCV that leads to it being able to escape the drugs, so then you would not respond to the drug at all. You will not know about that until your 4wk, 12wk PCR results.

With regard to ribavirin, you don't need to eat ribavirin with 20g of fat, but it doesn't matter if you do. People take them together with the telaprevir for convenience sake, it does not affect the potency of ribavirin. After telaprevir then you only need to eat 100cal with the ribavirins. I was 4wks und doing the same protocol as you and fingers crossed will be 12 wks und (I'll know in 2 weeks)!
