good and bad news and some questions

hi everyone,
first, i would like to say i hope everyone is doing good and either defeated the dragon or in the process.  i had shot 13 today. most know here i started sept 1, and was taken off tx for 2 weeks at the ened of sept due to sides. i was put right back on at 800mg of riba from 1000 so the doctor is cont. my lab work as i never stopped. although, in the end, i will have 3 extra shots to make up for the 2 week break.  anyway, i had my first 3 month PCR test today wich is great if all goes well. unfortunatley, i had to have my first shot of procrit last week, as some know, but today i was told i have to have neupogen shots with procrit. the doctor didnt have any there so they will have to get pre-authorazation from my insurance. i am much better than the start of tx, but have to stop at every floor to catch my breath and rest. have chest pains off and on, but not sure if its tx or gas. my doctor said today, the higher you start on tx, the better chance you have to clear. although, alot of people told me as long as you stay at 800mg, studies show theres no diffrence. i am currently 144pounds 5ft 6inches, a midget, so i hope it works. is it true if i am not undectable next week, chances are its not workin? i started with 399,000, they say low and i am 3A. liver test are all good and ultrasound was good. any advice would be helpful. she said my PCR could take 2weeks and the test only goes to 500, and the nurse said thers no other test anywhere that goes lower, i said i was told diffrent. well again, thanks for listening to my whining.

Hi Sally, I did 6 months of tx. Undetectable from week 4. Undetectable 4 weeks post tx, too. Thought I had it licked.
I don't want to jump to conclusions but I suspect what Bob K says about the high ALT may be true. I will get both the quan and qual though. How you doing? Peace Larry

Don't know what an Ultraqual is. Is it a qualitative PCR?
I have had this lab screw up a couple of pcr's. Once they did a quantitative when a qualitative was ordered. This time they did a qualitative but it is the kind that shows <615 copies and didn't specify the number. I don't know if it is the lab or the insurance company. I have orders for both a quantitative and a qualitative this time. I guess I'll know in a couple of weeks whether it is a false positive or not. Frustrating! Peace, Larry

False postives for qualitative tests run around 24%.  The rise in ALT is a little damning to hold out much hope for the false positive.  Sorry to hear the news...been there ...done that...not fun    :(

ps. definitely have a quantitative done to check for virus and viral load.

Gosh Larry,  I'm really sad to see you back with this news. As a 2b, how many weeks of tx did you have? As you know, this genotype is usually a good responder.

I guess this means another round of tx. It sounds like your biopsy indicated a good liver...good news.

could you have reinfected yourself? toothbrushes, razors, clippers? if so you would probably clear again, wouldn't you? i was told someone here's story that they had to do tx 2x's before they had a 6 month svr.they were clear after the first time as well and it returned and they had an easy genotype too,3a. i think thier story may be on "old hippy chick's/OHC'S " web site.  they went on tx for longer amount of time the 2nd time. then they sustained responce. i pray you will have better news though. Early response? I was undetectable the first month and through out tx if that is what you mean? 6 months geno 2b excellent bx.
I'm soon to be 52. Have had some unspecified eye difficulty during and since. I have a vertical imbalance that has got worse by about 4 clicks in the past year. Also some blurry vision, tired eyes, like strained a lot. PA recommended I see an opthamologist instead of  an optometrist. Something about a slight incidence of retinopathy in the literature and is better detected when after the eyes are dilated. I hope you both are successful in "kickin the dragon's butt." Don't know if I'm ready or willin to do another round with the peg/riba combo. Peace, Larry

I'm back, so's the hep c, apparently. 3 months post tx and it appears the dragon has once again raised it's ugly head. Where's my trusty steed. It's jousting time again. Damn! What's the likelihood of a false positive? How 'bout if the ALT is 122? Peace, Larry

I had a false positive at 6 months. I had prescriptions in hand, my little PegIntron kit with video, and laying the groundwork for another year of being slow (anemic) and in pain.

The ONLY reason why I had another PCR done was the lab went and performed their standard PCR when the doc (with me watching over his shoulder) wrote orders for an UltraQual. Had the UltraQual and then a third PCR (negative) to check on the previous test.

False positives do happen.

What a bummer!  Total bummer.  Well, dude, you do have some good company.  All the more reason we want the silver bullet to be found ASAP.  I'm just getting near the finish line, late January, hanging in there; we started on the same day back in February.  Best wishes, hope you get some clear answers and options...

Larry, I am so sorry to hear that you relapsed.  It's hard to know what to say.  I know that you are still adjusting to the news and not yet making plans but wanted to ask if you had an early viral response.  From what I have read, an EVR in someone who relapsed after tx is an indication that retreatment may work.  Take care.

Hello everyone! I'm back. I think the dragon may be also.  I was having a great time until they showed me my 3 month labs yesterday with a positive qualitative PCR. I am hoping it is a false positive. My ALT is 122 and that is higher then it has ever been. One month after tx in Sept. qual PCR was undetectable and ALT was 45. Hemoglobin A1C went from 4.6 to 5.5.  I've gained about 12-15 lbs and been pretty busy with work and getting my house ready to sell. My girlfriend and I were joking at Dr Cecil's office. I feel OK. I didn't expect it. Wasn't even considering it. I was looking forward to my 6 month PCR so I could start looking for another job or start my own business and get out of the snake pit I currently work in. I figured 6 months svr and I'd risk going without insurance. Now I fear I may have to remain there. This for some reason seems more intolerable then the Hep C. Undecided about what I will do so far. All kinds of thoughts rattling around the brain pan. I intended to stay in touch but got caught up trying to make up for lost time while on tx. Peace!  Larry

Boy, that nurse is a real doozy, huh?
Everybody says look for a 2 log drop to see if it has any chance of working. Drop 2 zeroes off your viral load, that's 2 logs. In your case you want to go down to at least 3990.

Hi Bill,
You need 2-log drop at 12-week PCR to continue,
e.i. baseline VL 400 000 UI -> 2-log drop <= 4000 UI. If your PCR > 4000 UI, your chances of SVR are 2-3%.
Then, as type 3a you only need 800mg of Riba. It's standard for Genotype 2 & 3.
