I have had the same problem. Long courses of predinisone are a definite danger in my opinion. A shot to a joint, typical for reducing inflamation in hips, knees, backs, etc. are a different thing since these treatments do not go into your blood system mainly, and is gone in a short time.
My hepatologist says it is OK for me to get a cortisone shot in my hip. I am on immunosupressants for transplant, also. So I would guess that if it is OK for me it is likely to be OK for others without compromised immune systems. See your doctor about this one to be safe.
I probably would not do it while on TX, though.

Cortisone has anti-inflammatory properties, and could suppress inflammation in the liver but has the adverse effect of increasing levels of virus in the liver, and reducing the chances of responding to treatment. It is not likely to help your liver in the long run. I note you have moderate fibrosis. You could consider treatment, but would need to consider whether you could reduce cortisone (in consultation with your endocrinology specialist) while on treatment to maximise the chance of a response. There is a blood fibrosis test available via lab21; their tel number is 01489 898600

In the situation of intravenous or oral steroids, HCV RNA level or HCV replication can increase significantly and is probably not beneficial for the HCV infection over the long run. However, local use of steroids, like the cortisone injection you describe, is probably OK. However, your doctor should aim to limit the dose of steroids (to prevent absorption into the blood stream at high levels). I assume that you have used shoe inserts - they can really help. Also, stretching excercises for the heel can work too.
