Please help with results.

My older brother is 50 yrs old with Hep C.Probably had for 25-30yrs continues  to drink beer on a daily basis (3-5 beers a day) Here is the Liver function test results
Platelet Count 109 , Red Blood Cell 3.97 , Trig. 162 , Albumin 3.1, Alkaline Phosphatase 153, ALT 59, AST 86
Doctors have not did a bx.yet . Is it unlikely they need to with this result?
Does this seem to be a severe case of cirrohsis?
Does it look like he is at the end stage of cirrohsis?

As his AST is higher than his ALTs and he has low platelets he should have a biopsy
He has signs of Severe Liver Damage. Whats his GGT?

You really can't tell a bad case of cirrhosis simply from blood tests. His ALT and AST elevations do not mean anything regarding the amount of liver damage a person has developed. His AST/ALT ratio shows cirrhosis as does his low platelet count although I'd be more concerned about the elevated Alk Phos (although this can be fatty liver) and his low albumin. There are classes of cirrhosis called Child's Class A, B or C. When cirrhosis is compensated and the liver is still doing it'sjobs, it Child's Class A. A person can live for a very long time with cirrhosis stable as Class A. Once the liver starts to decompensate (fluid in the belly, blood taking too long to clot, encephalopathy, varices in the esophagus) the person should be evaluated for a liver transplant. However, your bro drining will take away any chance he has of being listed...at least until he stops and is sober for at least 6 months. And yes, it is unlikely he needs a biopsy unless the doctor thinks his lab values are more from drinking than hep C but that is doubtful.

if he doen't stop drinking he'll be in real trouble...i got my enzimes down to  51 / 86 and thought i had done really good...thoughs are just a little high from normal...i'm not sure on the other stuff though..but get him to quit the drinking cuz he'll be going down hill from here if he doesn't..find out what geno type he is..then get a biopsy if you want to find out the damage...good luck...billy
