elivated ALT between 60s -110s need help/advice

hi

i am 31 from uk, i am neg for hep viruses pannel, my ALT levels are remaining between 60s and 110s for nearly 6 years

i had an ultrasound done  2 years ago and my liver was fine, no fatty liver i used to drink on some weeks ends on and off,i did fully stop for 2 years. Anyways 8 months ago i was feeling ill and weak, dr advised it could be my liver causing the weakness which i am still having, my ALT was still in the 80s then, 2nd ultra sound done a few weeks ago shows fatty liver and inflamation (note i havent had a drink for nearly a year now and was only a social drinker), and my case has been closed. The dr didnt seem to be concerned, saying i havent got the dangerous hep viruses so its nothing to worry, i am told to exercise and lose alittle weight and have a healthy diet.

my question is, my ALT has been raised for many years, only recently i have had fatty liver confirmed on ultrasound, which wasnt fatty 2 years ago confirmed by the older ultrasound.

1 is it wrong for the drs to assume its only fatty liver that caused the raise in ALT even though previously it was raised when there was no fatty liver?

2 my iron has been checked and i have had a antibody test too,neg on hep pannel, if it wasnt fatty liver causing the raise in ALT what could be causing it?

3  could inflamation in other parts of the body cause elivated alt, like the prostrate? as i have suffered from prostratitis, which is stil under investigation.

thank you
tony

hi

i did go docotrs concern of my digestive problems, i did find out i am positive for h.pylori, will be getting that treated soon, my concern was the chronic dihorrea, stomach duodenal ulcer, i have seen blood in the stools on and off, i was treatd for IBS for a long time with no checks done, The bleeding seems like bleeding hemorrhoids anyways, the Dr has taken me seriously and i will be having blood tests done,
I have seen a post, mentioning a mild ALT can be a sign of some form of chronic liver disease, such as metastasis of the liver, cirrosis, by Manual of Laboratory Diagnostic Tests by Frances , 2nd edition

so i could very wel have the colon issue which i am getting investigated, whcih could be causing the mild/moderate elivated ALT for soo many years withought no explanation, i ahve had all blood test, apart from blood test for cancer/tumour checks, not had a biopsy of the liver.
i have had:
fbc,clotting profile, u&e,fasting glucose and lipids, alpha-1at, immunoglobulins,ferritin,
ttg, autoantibodies, hep b, c, ultra sound shows inflamation of the liver which has now become fatty, was not fatty 2 years ago, i have had raised ALT levels for many years.

you do have a point about if it were cancer int he liver, its been too long now, maybe:

metasisis of the liver takes longer?
speed of cancer spread in liver maybe slower?

thanks for the reply

i think colon scopy should rule it out, which im going to try getting, so it it were secondary cancer of the liver, not only alt would be raised, also a person with it wouldn't live more then 2 - 3 years? i mea i have been drinking alcohol binging on and off in the past only 2 or 3 times in a month, somtimes i have been off it for a year,i havent drank for over a year now, could this also cause raised alt, maybe takes years to go normal once stopping drinking?

thanks

If you had liver cancer that long, you would probably be gone by now. Plus other markers would be elevated.

There are many things that can cause elevations in liver enzymes and many fo them are quite benign. A liver biopsy would probably be helpful if you can get one approved in the UK. Things like taking too much tylenol or some other meds can cause elevations. So can things like auto-immune hepatitis. Of course there are other liver diseases besides hep virus. If you are still cponcerned you might want to pursue a biopsy to get to the bottom of this once and for all.

hi

my ultrasound many years ago even though ALT was raised i had no fatty liver it is only last year ive got fatty liver with inflamation, so fatty liver really doesn't look like the cause of this raised ALT.

would  cancer in the liver show up as a tumour? or would a tumour not be visible, as the ultrasound hasn't shown a tumour in there?

are there any other bood tests you can reccommend for liver or colon cancer?

i will be goin to my gp to address my concern of colon cancer. as i have a history of chronic diahrrea, hemorrhoids, stomach craps and on top a history of colon cancer i my family.

would love to see what your suggestons/advice is on this

thanks

apart from being slightly overweight you seem pretty healthy and active, if your liver conditions are not hep c related or alcohol related it is usually diet and exercise, but you seem ok there too, maybe you should just keep an eye on things and get a lft done 6 monthly and review then..your prostate issue could be a cause i really dont know there , i hope you get it sorted soon..
