24 weeks Pregnant HSV I/II IGM positive

Dr H & Grace,

I read tons of threads that mentioned to ignore IGM tests but here is a rather strange situation that may compel one to consider these results. If not what is the best way for a pregnant woman to confirm a recent infection?

I went for my 20th week ultrasound check up and they found an echogenic bowel and the doctor administered us to go for the TORCH test and here are the results:

CMV - IGG = 16.9
          IGM < 0.9

Parvovirus IGG = 5.8
               IGM < 0.1
Toxoplasmosis : IGG/IGM not detected

HSV1 - IGG <0.1
          
HSV2 - IGG <0.1

HSV I/II IGM < 2.08 H

Now we went for a repeated ultrasound and see that the lateral Ventricles are enlarged and measuring 10mm (unilateral) . Could this be due to Herpes infecting the fetus and causing neonatal complications? Also neither myself nor my spouse had any sort of HSV symptoms. We had an unprotected sex just two days before we went for the blood work that came back with HSV I/II IGM positive.  I am literally distressed and panicking with this situation. I now went for an amnio, HSV PCR for both myself and fetus from amnio and waiting for the results. I also took the verifi test for down syndromes and it came back normal.

Has anyone been in similar situation? appreciate your responses.

that is why we don't recommend herpes igm testing in adults - all it does is further complicate things needlessly :(

I wouldn't bother with any more herpes testing at all. no real reason to do so at this point. no receiving oral sex from hubby until after baby is born though!

We finally received our husbands test results for all the STDs and HIV and the results are below:

HIV, HEP-C, Chlamdia, Gonorrehea are all negative.

HSV1 - type specific IGG :  3.70 H
HSV2-  type specific IGG :  <0.91 Negative

HSV1 IGM  <1:10 titer
HSV2 IGM  <1:10 titer.

Does this technically mean I was prone to be infected with HSV1? while I understand there is a potentiality of being infected, does this IGM of 2.08 that I got kind of inclines to a recent infection of HSV1? But as mentioned the results from the amnio came back as negative of HSV1/2.  I would have cared less if it was not for my pregnancy as it is HSV1 and my hubby suspected it right from beginning that he may have HSV1 from childhood and he realized that he might be infected with HSV1 only after my IGM results came back and when we started looking at what all these HSV is about. The pics of cold sores made him convinced that he was infected which is now evident from the results. But what is bothering me now is how harmful it could be to my baby if I have to go on IGM results. I will re-check myself for any IGG antibodies again in June but trying to find some answers on the possibilities of getting this infection and I should mention that it was just a regular intercourse that we had back in March. We werent really sexually active during this entire pregnancy and have been careful enough except for that one abbe-ration. Also now on the baby side the brain ventricles turned back to normal measuring 7 & 8mm which is a big sigh of relief. Overall this HSV IGM has caused quite a roller-coaster disturbance in our lives.

well if baby has herpes, so does momma but other than that, there are limits to the pcr testing on amniotic fluid.  herpes would not be actively circulating the entire pregnancy to be detected.

He gave his tests and we are waiting for his reports. Also, the genpath labs who did this amnio test for HSV mentioned that this result applies for both mom and fetus DNA. They said this because the doctor's lab took my blood work for HSV PCR and Genpath rejected to test on the blood and mentioned that amniotic fliud does check mother's DNA. I am trying to understand these tests and it is way beyond my knowledge. Hence seek your expertise.

oh it was from the amniotic fluid and not your blood?  sorry about that, I assumed it was blood testing for you.

pcr testing of the fluid would not reflect infection for you but if the baby was infected with herpes within the last 2 weeks or so, it might be helpful.

did your husband get his herpes testing done yesterday?

That is confusing Grace,

So do you think this is a wrong test? Well this a test performed on the Amniotic fluid that was taken from my womb to do FISH and Karyotyping and this test was also give to do Herpes. I am concerned now. While I get some answers since my hubby has given his tests as well, I just went to the JFK hospital in NJ and the perinatal doctor also confirmed that this is the gold standard test by doing the HSV PCR on amniotic fliud to ensure both mom and fetus are not infected. I am not sure if I gave wrong information earlier or do you think with the info now you still believe this is a wrong test?

we don't test for herpes with pcr blood testing.  your provider erred in ordering that testing :(

You are right on it. My IGM results were false positive. I got my results on PCR-DNA( which I believe is 100% accurate) test on HERPES 1 & 2 and they both are negative. Thank you so much for your support. But the fetus issue on Ventriculomegaly is still there. I know this not an appropriate place for question on it. Could you please guide me to the group where I can post me question on it?

you can have neonatal herpes infections cause hydrocephalus but there are also many other causes of it.   you testing negative so far is on the way to ruling out herpes as a cause. not likely that you would be negative for herpes, contract it genitally from hubby and not have an obvious ob by now.  

you need to totally disregard your igm results. they really don't tell you diddly squat.

no, antibodies wouldn't show up in 2 days.

My husband took his appointment for Monday the 29th.

I meant the Brain Ventricles.Has it got anything to do with Herpes?

Also whats bothering me I gave my blood work just two days after we had sex. Can the anitbodies show up within that short time frame? Also Given my past CMV infection, can this cross react with HSV IGM results?

no it's not accurate in pregnant women. it has a very high false positive rate.  

has hubby been tested for herpes to know his status?

are you referring to the heart ventricles or ventricles in the head?
