Quest Diagnostics Test (34982)

Does any one know what this test is for? I had an outbreak in my finger and my doctor ordered a PCR DNA HSV test on serum which came back as negative for both types. The lession has come back and now she is ordering Quest diagnostic test (34982 Custom) what is this for?

Grace, do you think i can get a pcr on that rash? please check the picture... does that even look like something i should be worried? I have been getting that type of rashes quite often for the last few months. They appear suddenly and last for about a week. Always almost at the same place.... not exactly but very close.

no reason for more blood testing.

be seen every time you have any symptoms for proper exam. it's time to move past herpes as a cause and it's wasteful to continue to seek out blood tests at this point.

So the biokit at three month mark is enough for me to believe that I didnt contracted HSV2 on that encounter? I am aware that HSV2 in the mouth is rare and that it cause little shedding... is that true?... I plan on getting tested again to include this possible exposure with my finger. I am also aware that whitlow is also rare and that it is mainly contracted by fingering an infected female and not very common by just making out... is my information correct? One more thing I forgot to mention... when i finished touching this girl... my hands where very itchy... extremely itchy. It took me about 2 hours to get home and wash my hands. Does that gives you some clue? My doctor is reluctant to refer me to a dermatologist... At this point I am not very concerned but definitly want to get informed.

this sounds far more like a contact dermatitis than herpes as we've discussed previously. next time you have symptoms, try to see your dermatologist promptly for better help. I wouldn't bother with herpes testing unless they really feel that it looks like herpes.

I guess I will never know that. I have always tested positive for HIV1.  I get cold sores in my mouth about once every 7-10 years. In fact my positive came as high as 6.8. I tested about a year ago with a type specific IgG 2 test. HSV2 came as .57. The rash in my finger now is a little bulky no pain, no nothing (after 6 weeks, peeled off at 3 weeks)... it is just there, banished (not as red as before). I guess I will wait to see if it comes back but to my understanding... I need to get a blister to get a culture or PCR done, is that right? I only get rashes... never blisters.

your biokit testing was fine to have done.

it's unfortunate that they didn't do the pcr on your active symptoms.  

since hsv1 can be the cause of whitlow too, follow up at some point with a type specific herpes igg blood test that looks for hsv1 and hsv2 for a little bit better answers.   the only way to know though if you test  + for hsv1 on the test if it's what caused your finger rash would be if the rash reoccurred and you could get that pcr'd next time.

you know... my head is becoming a big knot. I just posted a question on the herpes forum. This is another concern I have. So now I dont know what the heck is right. I always thought that three months was the right time to get tested. Now I have seen some suggesting 4+ months. I used to believe that the accuracy of the test was enough to give a difinitive result. I just read an article a few days ago from the NIH that the accuracy of serology tests in one study are very low (about 40-50%) compared to western blot. And that the valume of false positives are high. Even the CDC dont recoment to be tested if no obious signs are present.

Yes it should have been done from the lesion in my opinion.
my 2 cents. I mean you can check for antibodies and if you know you never had Herpes type 2 for sure then a blood test could be ok but it isn't as specific to the site as it would be take is right from the area. You know doctors don't really get that much microbiology in med school except for infectious disease docs. They are really good. i worked for one for several years.

yeah... that is what she said. She ran the PCR on serum which I believe is not accurate. So I have no idea what she is doing and she don't seem to be willing to explain me. Thank you for your responses. I will continue to see her and have a second opinion at the same time...

There is a type of Herpes called Herpetic Whitlow usually type 2 Herpes that is on the fingers. That is probably why the doctor did the herpes PCR which turned out to be negative. But I have no clue why they are testing you for encephalitis.

At this point I'm more worried about the credibility of my doctors abilities and knowledge in medicine. I am completely fine. I just got a rash kind of a bump in my finger and she runned a PCR Test for HSV on Serum which came back negative and Grace said it is useful if done on blood. Now she is ordering this test which for what I have read it has nothing to do with a small bump in a finger.

Encephalitis, can be a swelling of the brain due to an infection often a virus or could be a meningitis. Did you get a MRI or CT scan? Do you have any symptoms such as unsteady walking, headache, fever , swollen lymph nodes? A lot of things can cause this. It can be very treatable and they are probably trying to rule it out or see if you have it due to some symptoms you have. It doesn't mean you have it until the test comes back. West nile virus can cause this for example. Have you been bitten by a mosquito? You don't know for sure you have it and it is usually treatable. Please try not to worry you don't know yet what the results are yet ok?

I actually found a test for that code with Quest and it is to detect encephalitis... I have no idea of what that is nor why she suspects that... she is scaring me.
