Question about lab work order and result forms


It has been a few weeks now since I tested positive for HSV2. I have had no symptoms that I readily recognize and according to my IgG value (5+) I have had it for some time. I am dealing with it my own way.

Some things have me concerned though. I was looking at my forms. The one I took with me to get tested and the one with my results. 

My name, phone, email and address were the same, but all the other stuff wasn't. My ID number, account number, control number, Physician and their number were all different. I am not trying to get my hopes up, but shouldn't they all be the same? At least my ID and account number. 

I did it through an online testing service which had me goto a Labcorp location. I cannot find a number for Labcorp and the online service just says it is all in the same account. 

Should all the ID numbers and stuff be the same?

It was a test ordered from stdcheck.com. The form I took to a Labcorp location, was a Labcorp form. I got the results back from Labcorp on a Labcorp form and all that was different. Why would the same company use different numbers is what has me a little confused.

Hi, not necessarily. Different companies log patients in with their own numbers. Tell us more about the test you had?
