If it's no longer the rash that is the issue then continue to follow up with the urologist.
they should be doing urine testing at this point.
this doesn't sound like a herpes issue from what you've described.

If you still have open sores in your urethra, you can ask the doctor to take a culture and test for HSV-1 and HSV-2. Otherwise, if the doctor / dermatologist persists that it is a yeast infection/fungal infection, ask them about your treatment options for the lesions in the urethra itself.

Grace is right: it sounds fungal. You should follow up with your GP to talk about the urethral lesions. If it's fungal and inside your urethra, obviously a topical cream / anti fungal cream isn't what you need right now.
They gave me one called ketoconzoe something like that.. 2 x day 2 weeks, which is fine i did the cream but the burning is inside not on the outside. Today I am fine in the morning with little burning, now its noon and the burning is a little worse... by tonight i already know it will be bad :-( By the way thanks for your help.

Thanks for the update and I feel a little better about what this might be. The Urologist has not taken any tests I dont know why, he thinks it might be related to low testostrone in my blood. I am not sure what kind of test to ask him to run for fungal infection.
Odds are you don't have herpes causing these symptoms.
You have had multiple antibiotics now ( literally throwing things at you to see what works I believe ) so odds are you do have a fungal infection going on. talk to your provider about using the topical cream ( you need to apply it 2x/day for a full week after the rash disappears or odds are it will just come back and sometimes it literally takes weeks of using it to get rid of the yeast issue - sometimes changing creams helps too ) as well as oral diflucan. it unfortunately might take awhile to get rid of this fungal issue :(

Alright, how long have you had symptoms? Depending on how long these urethral lesions have been present, it may or may not be Genital Herpes.
Also, the fact that every 3 or 4 days there is no pain is indicative that it is not herpes. Herpes presents with outbreaks that last 1-2 weeks and disappear completely thereafter, with no remaining symptoms. There are no "breaks" in between symptoms for Herpes. Also, herpes does not get better or worse over the course of the day. This points towards irritation: overnight you aren't irritating the area, but by afternoon and nighttime you've exercised and jostled the area quite a bit. This could simply be irritation or a fungal infection made worse by friction.
GH can present in the urethra, but generally presents elsewhere on the penis as well. You should see a dermatologist / your GP immediately and get a swab/culture of the lesions in your urethra. This will determine if you have contracted HSV-1 or HSV-2.
I wonder why you've been prescribed azithromycin...that's for bacterial infections, like upper respiratory infections
