need your help

A 70 year old patient with h/o splenomegaly, generalised weakness,on and off fever having following blood picture:
TLC- 13500
DLC-  N:42% L:12% M: 2% E:2% B:4%
         Blasts- 8% Promyelocytes- 1% Myelocytes:14% metamyelocytes:3%
ESR- 54
RBC count: 3.5 million
platelet count : 1.9 lac
wat cud be the probable diagnosis??
and wat will be its prognosis??

I am not a doctor, therefore I can't give a probable diagnosis.  Every patient is different...therefore a prognosis would be difficult to give as well.

Has the patient seen a hematologist or an oncologist?  However I am curious to hear what the patient's doctor says about the blood work.  With the spleen being enlarged...it's a safe bet to think it may have a lot to do with the blood work.    The spleen filters blood and removes old and damaged red blood cells, bacteria.  It also produces lymphocytes that produces antibodies and assists immune system.

What do his/her docs say is causing the enlarged spleen?
