The standard CMP blood test (comprehensive metabolic panel) will provide a good starting point in which to form a diagnosis. The test includes:
o Albumin: 3.9 to 5.0 g/dL
o Alkaline phosphatase: 44 to 147 IU/L
o ALT (alanine aminotransferase): 8 to 37 IU/L
o AST (aspartate aminotransferase): 10 to 34 IU/L
o BUN (blood urea nitrogen): 7 to 20 mg/dL
o Calcium: 8.5 to 10.9 mg/dL
o Chloride: 96 - 106 mmol/L
o CO2 (carbon dioxide): 20 to 29 mmol/L
o Creatinine: 0.8 to 1.4 mg/dL **
o Glucose test: 70 to 100 mg/dL
o Potassium test: 3.7 to 5.2 mEq/L
o Sodium: 136 to 144 mEq/L
o Total bilirubin: 0.2 to 1.9 mg/dL
o Total protein: 6.3 to 7.9 g/dL
So the doctor will have data covering the liver, kidney and electrolytic balance in the blood.
Given the history of autoimmune disease in your family, an anti-autobody profile should be developed as well.
o ANA, SSA, SSB
o ANCA, AMA, ASMA
o Complement profile
o Immunoglobulin-G subclasses (IGG)
These are just the main tests. And your doctor should make the decision of course, we're not doctor here, just patients like yourself.
Given the extended time that you've have an elevation, I think a more in-depth look is needed. If your doctor thinks you simply have a fatty liver, then great, but he needs to let you know, so you can stop worrying. But if he doesn't really know what's up, he needs to do something.
Thank you sir..so what is the test that you recommend for me to take..i'd like to ask my doctor about that..i'm having these conditions for about three years yet still my doctor dont recomend any new test for me... he just ask me to control my diet and exercise.. for three month i did just that my ALT level went down to 67..my doctor also forbid me to take any statin for my cholestrol .. 3 of my family members had SLE (lupus)..so far i dont have any symptom for that..by the way I am 33 years old and dont have any medical condition accept that..
Has your doctor told you what your actual diagnosis is? Simply saying that it is inflamed is vague and insufficient. There must be some cause to your liver's stress, the elevated enzyme levels indicate that your hepatocytes (liver cells) are under attack. It maybe simplybbe a fatty liver, or maybe a yet unidentofied virus or parasite, autoimmune, drug, etc .... But the source of the injury needs to be identified in order to find a truly effective treatment. And simply testing your ALT blood (sgpt) levels isn't enough to do a proper differential diagnosis. We'd like to help you but there isn't much to work with, more data is required.
