I will go send it to you now, but ask that you *not* put the doc's name in public here, to protect his anonymity in public.
He is a very good MD, and I would not hesitate to be treated by him again. He is well known in the Lyme world, but just as a rule of thumb, we avoid naming names here in the open.

Are you familiar with the 'private message' function on this website? I can send you a recommendation there, since we keep our doc's names out of the public area of this site, to protect our Lyme docs -- in some jurisdictions, the Lyme docs need to keep very very quiet to avoid the state and local medical boards from trying to shut the docs down.
