You are correct in pursuing a full cardiac workup as this is an important thing to diagnose

another possibility is focal seizures - this could be consistent with the episodic paresthesias, and strange smell or taste
An EEG (an electrical brainwave test) and an MRI of the brain should be done to evaluate this

It could jus be muskuloskeletal, (and for instance you are lying n a nerve during the night), but this is a diagnosis of exclusion

Good luck

I'm exactly in the same boat as you.  I mean all the same symptoms.  I've been to the ER twice and both times they said my heart was fine.  I seen a Cardiologist who ran a bunch of blood tests, EKG's and a stress test and he said my heart was fine.  The only thing different from you is I sometimes have all the other symptoms without the chest pain.  My GP keeps thinking it's anxiety and keeps trying to feed me anti depressants which keep making me depressed and I stop taking them.  When I have these "attacks" my mouth sometimes gets super dry.  Yours ever do that?  I have to say the last couple of weeks have been better, I mean I haven't had any of the attacks, but I still don't feel right.  I now have a permanent slight ringing in my ears and I have a lot of floating spots in my vision.  I see my GP next week and I'm going to ask him to refer me to a Neurologist.  I'll let you know what the finding are.

Just to conclude my concerns... I've been to my PCP, the ER, a Pulmonary doc, had an Endo that looked good and did a sleep study all in these past weeks.  I'm not sure of my best direction, but when I'm having these episodes I feel a bit desperate.  Last night was another night of spasms (almost tick like and more noticable in some sleep positions than others).  They are normally preceded by some type of chest pain, twinge or something noticable that seems to rush to my head alerting me of something... which occurred around 9:00pm last night.  There are some good nights and for stretches of 3 or maybe even 4 days, but they don't seem to be leaving for good.
I'm on Prilosec, with NO caffeine, chocolate, alcohol or any of those summer pleasures I'm used to enjoying.

Any help, or thoughts, would be greatly appreciated.  Thank you in advance.
