Pelvic ultrasound

What is my next step:
Findings: Enlarged heterogenous leiomyatous uterus with the uterus measuring 16*8.8*11 cm. Multiple circumscribed low attenuation masses consistent with eiomomata distort the parenchyma. A dominant fibroid extending posteriorly off the serosal surface of the fundus measures 5.5 * 5.3  aprox 4 cm. Edometrial complex obscured on the transabdominal image and endovaginal limages. The images of the endometrial complex on endovaginal exam measuring the endometrial complex at 1.6 cm. The endometrial complex was oblique. Best estimation is 8mm. Bilateral ovaries were not visualized, obscured by eht enlarged uterus. Physiologic fee fluid in the cul-de-sac. Diagnosis: Acute fibroid uterus, pain and bleeding...........Plz Help me understand
