Would the ca 125 value elevate this high in a benign condition? The GYN/ONC is fairly certain that there is no cancer judging from the mri and u/s. However, I still need to remove the l/o as a precaution.

I am glad to hear that you will be meeting with a gyn oncologist.
Even with normal xrays, I think the gyn onc will tell you that you need surgery to fully evaluate the abnormal and increasing CA 125 value.
There are definitely benign causes of CA 125 protein excess such as infection, active liver or heart disease, and inflammatory conditions such as diverticulitis.
But at this point, you need at least a laparoscopy to look inside the abdomen
