pregnant 1st trimester - complex ovarian cyst malignancy suspicion

I am 8 weeks 4 days pregnant and have just discovered through an US after cramping in my left side that my left ovary measures 3.5 x 2.2 x 2.7 cm and attached to this left ovary is a large complex solid and cystic mass in the left adnexa, measuring (5.4 x 2.4 x 5.4 cm) with evidence of a large mural nodularity, the largest of which measures (3.4x2.7x3.0 cm) There is no hypervascularity. The right ovary is unremarkable, measuring 4.7 x 2.4 x 3.2 cm.  No free fluid is identified.

I am seeking additional advice to support the advice I have already received or explore alternative options for treatment.

I have a follow up ultrasound scheduled in one month to observe the cysts size.  If it has shrunk it is likely that it was blood clotting that was showing up on the US and the cyst will continue to shrink making this a functional, albiet unusual cyst.

If there is no size reduction or growth shown through the US I am being advised to have surgery in my second trimester of pregnancy to remove the cyst so that it can be checked for malignancy.

I would like to know what the risks are associated with waiting a few more months to see if the cyst will reduce in size.  What are the chances that a cyst will reduce in size? What is the optimal time to have an invasive procedure during gestation and why?  Are there any steps I can take to encourage size reduction or elimination naturally?  Can accupuncture be used safely to reduce size of cysts?  what can a gyn/oncologist tell me about the cyst without surgery?  What is indicated by the observation of no free fluid is identified?

Hello Tamara,
I had the surgery too, two weeks ago. The cyst was not benign, and not malignant, it was borderline, as it appeared also on ultrasound.
The good news is that the baby is fine and growing and hopefully I will deliver on due time and he will be healthy.
I couldn't have chosen to keep the cyst as the borderline tumors tend to turn into malignant over time and six months till delivery is a long time I think.

Hello, I understand how the three of you must b feeling right now as I went through the same thing in 2005.      After several yrs of trying to conceive I was to have an ultrasound.  I learnt that I had a cyst on each ovary but were considered small and believed to have no conection to my inability to get pregnant.
They were to be monitored.  After several mths and I believe two  TVUS the doctors believed they should be removed via laparotomy.  It was going to take a mth or so before I could get in for the surgery....in the meantime I found out I was finally pregnant!  As much and for as long as we wanted to get pregnant this was not the time for it,  but they cancelled the surgery and said we could wait till after the birth of our child.but I had to pretty much be on bedrest.  
At 15 wks I was in tremendous pain especially with any bending, walking and movement basically. Went for an ultrasound and found that in just a couple months my cysts had grown to 11cm and the other 9cm.   They worried that thye may rupture,  put to mush pressure on the baby, and worse of all be malignat.   So I was scheduled to have a laparotomy 3 days later on Dec. 23/05....
.....they did not turn out be malignant and I ended up gving birth to my beautful healty boy on my due date.
I was told that 16 wks gestation is the best time for surgery as the baby is not developed enough to endure us being put to sleep and medications and such and any later the babies size could cause the cyst to rupture causing infection which with some cyst cause something like appendicitis.
I remember how scared Iwas, and all the emotions I went through, but I just want you to know I  have gone through it and it all worked out..  I will be thinking of the three of you.  I'm just so happy you all have found a place where you can tak with one another, as I'm sure it really helps.

Wow...I am glad I found the both of you!  I too am going thru the same thing...

On Wednesday, I went into my OBY/GN's office thinking that I was about 6 weeks pregnant.  When I laid down to get my physical exam done my Dr. said that she thought I was 2-3 months further along than what I had calculated based on my 'uterus' size.  When she fired up the ultrasound machine she pretty much told me it didn't look good.  There was no heartbeat and the 'baby' did not look right.  I have had a miscarriage before so I had tried to prepare myself for this possiblility before I went in.  Luckily, my Dr. is wonderful and sent me over to another office for a more in depth ultrasound.  That is when the rollercoaster ride began!  I was indeed pregnant!  Heartbeat and measurements all were great for a 6 1/2 week old fetus.  However, I also had a very large tumor attached to one of my ovaries.  This tumor has completely seperate from the baby but it was so large that it blocked it from view.  At that ultrasound the Dr. measured the tumor at approximately 12cm x 12cm x 9cm.  I was told this is something that had to come out surgically.  I had an MRI yesterday to try and 'diagnose' what type of tumor I had but it did not help.  On my MRI the tumor measured even larger.  My Dr. is now consulting a gynological oncologist to see if he thinks this is cancer based on my tests.  I had a CA 125 done also this morning.  I am so scared...not so much of having cancer but of all of the unknowns...I just want to know!!!  Then I can have a plan!  I am waiting to hear back from my Dr. to see if she is going to do the surgery (if she thinks it is not cancerous) or if the specialist will do the surgery (more likely cancer).  I am trying to wrap my head around all of this but it is so hard!  Every time I look at my almost 2 year old son I tear up...I am 30 years old...I am so scared!  I am also scared for this baby...if I have cancer what am I going to do?  Thank you so much for this outlet...I feel a bit better already just telling someone my fears!  I would love to hear from the both of you with any updates you have!

I am in the exactly same situation as you. My doctor discovered on US an ovarian cyst of approx. 5 cm which is attached to the ovary and I currently am 10 weeks pregnant. My doctor suggested a CA 125 blood test which came at a value of 38, with a normal of 35 units/ml. He is saying that surgery is needed and it won't harm the baby. He wants to take the cyst out, possibly the ovary too, and examine it. I am very scared at what effect this could have on the developing baby and I am really hoping the cyst will get smaller. The doc said it is ok to do surgery in the second trimester. I am really scared and cry all the time... I want to see another doctor for a second opinion and repeat the CA 125 after two weeks. Hopefully the result will be lower.
