Well, change can cause anxiety. Try to relax.
A case manager is a medical professional, usually an RN, who helps patients navigate the increasingly complex medical system. A case manager will review your case, match you with the proper resource provider, answer your medical questions, make sure you have proper prior authorizations for treatments, etc.
