T10 paraplegic. Messed up w PM need advice

I am a T-10 complete due to gun shot wound 6 years post.
So after being with my current Dr. for five years, I was called today and told that i was being discharged. Due to my past urine screen coming up for and "illegal substance". The crazy thing is that i am the one who asked for a culture to be done on my last visit, bout 2 weeks ago, to make sure a recurring UTI i had was completely gone. He Rarely Urine screens me. I feel bad because we had a good relationship and he even knows i smoke M. for quality of life issues and spasms, though i dont live in a M.M. state, he didnt have a problem with it, as i told him on my first visit.
The Problem is that it came back positive for something else. I think?. . I was offered something at a party the previous weekend (5 days previous) and decided to do some. It was a stupid mistake and is not something i normally dont do. I figured that it would be out of my system within 72 hrs. The testing they do at PM is just a dip panel test. I figure after being sent off for the culture is when something was found.
I do have severe pain and spasms 24/7. Two kinds really, The "on fire" legs an butt and physical pain in my back. I have tried many types of pain relief. The current meds i am on are an Opiate for pain (which has been amped up over time) and a Benzo for spasms. These meds help my pain A LOT and make it so i can live a decent life.
I am very scared of the future now and dont know what to do. I will have severe withdraw and be in so much pain that i will not be able to do much other than lie in bed.
I know the obvious answer is to find a new Dr. It was very difficult for me to find this one though. Plus, what Dr. is going to take a patient who just got discharged for "that"? I called a couple PM Drs. in my area today but they want a refferal from "my dr" for an appointment. I am now to be labled a "drug seeker". YES, I AM dependent on my meds. Otherwise Im in a lot of pain. Does that make me a drug seeker? Yes i messed up and did something i shouldnt have, Im sure im not addicted to it though and dont do it on any regular basis.
Has anyone been through this?...and willing to admit it on this forum? Or have any advice on getting a new Dr...or keeping my old one. Please save the "shame on you's" as i know i messed up.

Hi Bandodidit,

I'm sorry that you are going through what you are.  I don't have access to any Doctor's names as we normally don't give that type of information out on the Forum because what works for one MAY not work for another.

The only thing that I can help you with is the question that you are asking about what type of Doctor you should be looking for.  You should look on the internet OR the phone book for a Doctor that specializes in addictions.  They would be the ones that can help you taper and POSSIBLY start over once you are off of the illegal drugs.  PLEASE take it seriously when you get another chance.  I REALLY want you to get the help that you need!!!

So I have been in contact with a couple PMP offices in my area that would give me a self referal over the phone. I was contacted the next day saying that they would not take me as a patient:(??
I have been brainstorming of what the heck is going on here.  i had such a good relationship with my past dr. If he thought i had a drug problem i would think he would have refered me to a reahab or something. Something.... I just dont see him not willing to talk to me at all.
THEN I REALIZED, that my ex roommate whos drug addicted mother went back to jail for a year and a day for the umpteenth time, and was recently released about 48 hrs before i got notified i was being discharged.
My exRoommates got really pissed because i told a mutual "friend" that i was happy to have my apt. back to myself and not to have to deal with all her drama. Her VANITY is EVERYthing to her. She is bipolar and has done so much b.s. She blackmails her b/f for A LOT of money because he apparently had stolen @ 100k from a pawn shop he worked at. Now he works for a gold buying store which he also steals from.  I was soooo dumb for taking her in to my house, but i am a sucker for a girl crying telling me she needs a place to live. Plus I had an empty bedroom and coulda used the extra rent money...which did come in handy but not worth the headaches and constant drama that was brought around me from her.
Her mother knew who my dr. was because she had seen him before. I was scared that she knew who my dr. was because she is a drug addict ect. Having a year in jail doing nothing, could easily give her time to concoct some crazy story about me if her daughter wanted her to.  Seeing as the time difference between her getting out and me being notified of being discharged are just too coincidental. Im about 99 percent sure that is what happened at this point and my Dr. just used the MJ in my urine as an excuse to discharge me.  
As for finding a dr.
What type of dr would i be looking for if i needed to see an addiction type dr, so that i could get my meds.  Or should i look for a physcologist .
I know the streets are not the answer, nor do i have the money to spend on pm from the street that i SHOULD/HAVE gotten nearly free because im low income.
ughhh. I have been weening myself off with the remaining meds i have left, but my pain is much worse now because of it.

Hey Baron,

I hope that your Christmas was good.  I have an idea of your pain and situation as one of my jobs in a former life was to care for two guys my age, one a quad and one a paraplegic.  Please don't consider the street as a resource.  Your situation will only get worse if you do.  The level of care in jail is not good and that's just one of many possible scenarios.  Please get on the net, consult people here on the board whatever you need to do to find a good PM Dr.  This is crazy.  Good Luck.

The nurse who gave you the contacts is the nurse I was referring to. Maybe she can get your referred to a PMP.
The streets are not the answer as it will only add to your problems in a huge way. You will end up in jail and then you will have problems beyond your control.
I will send you the names of a couple great PMP in the Gainesville area.

i really dont think that would help me. As i was speaking with her, he was just steps away...on the other side of the door.  This is not the same nurse who called me to notify me of being discharged.  she was actually nice to me and at least wrote a couple pm drs on a piece of paper and gave me the form to write a letter to the dr. Its weird that he wouldnt see me. Maybe he "cant" because he discharged me.??
  Ive heard so many things from different people at this point. Ive heard there is a statute in FL that says if ive been seeing a dr for pm for over two and a half years that he must coordinate a ten day taper for me if im being discharged. i dont know if thats true or what.

  I NEED to get this taken care of fast!! Im not just someone with a bad knee or something.  I am a PARAPLEGIC i live my life in a WHEELCHAIR. My T10 is in lil lil pieces spread around where it used to be. I have a .45 in my liver still which can make me feel like i have non stop runners cramp.  I start every morning my digging in my rectum, with hands, to empty my bowels.<uggghh I am now Dependant on an opiate which is pretty much H.  The withdraws i would go through could kill me. Actually the sudden stop of Valium for my spasms could REALLY kill me,   I am low income and live month to month. I have medicare and medicaid secondary...which is hard to find a pm dr to take.
I should be free to do anything that includes relieving my pain and improving my quality of life.  This is crazyness.  Should i start looking on the street? Should i pack up and move to mexico/ canada?

I'm gonna put it very bluntly.  Once again, you let the gatekeeper stand in your way of directly speaking with the Doc.  I have experienced same with a Nurse who was on a power trip.  It's amazing what you can achieve face to face with someone you have a relationship with.  Do whatever you have to do legally bypass this barrier.  Don't be shy. Sandee1818. Yes i would like the name of that PM Dr. Please. Maybe you could PM it to me, im new to this board so i dont know if you can do that? Do you live in Gainesville? I know i messed up and my pain medication is a very serious thing to me. I have NEVER lost my meds or gotten rid of them. i need every single one. As i stated before I fealt like i was being prescribed JUST enouph meds to get me through the month. My pain is  also very hard to manage even for a dr. because many things contribute to central nervous pain. If its cold outside. If its raining. if I am stressed out,  my pain gets worse. I never know when its gonna get real bad and i need the extra few pills i have left over.
I know I am going to have to prove myself where ever i go from here, i will NOT make this mistake again and if need be i will stop smoking pot. Its gonna take 30 days for that to be out of my system.
BTW the nurse didnt exactly say they would refer me to another Dr. She just handed me a slip of paper with a couple pm drs on it. When i asked if the dr. would refer me to a new one, she just said sure, find one, and we will fax over your records.
The one i want to see says she needs a referal? he is a pm dr.

I also wanted to tell you that you do not need all of your files. Have only the test results of test you have taken and the last Doctor notes from the past 6 months. This should drop the price significantly.
I do hope you have learned from this mistake and will make sure this does not happen again.
I do not want to sound rude but these kinds of things are the reason why it is so difficult for us to obtain pain management. People who misuse binge, overdose, or
, lose drugs (stolen, misplaced), or even divert(selling, giving away drug, diversion to friends, family, or the street) make it so hard on those who do follow the rules and take the medications as prescribed and do not use illegal substances.
I know your aware of this I just wanted to make the point here for others who may find themselves in your position.

The nurse at your Doctor that discharged you said they would refer you?
If so let her refer you to a PM Doctor.
I would also suggest sitting down and writing a formal letter to the Doctor and explain what happened and it will be included in your file.
I think you will be much better off with a PM Doctor verses a PCP also. I know of an excellent PMP in Gainesville if you would like the information let me know. You need to get on top of getting an appointment with your medications running so low. This is important as you know. Call the nurse and let her know that you need an appointment asap.

Sooo, I showed up at his office to see if i could talk to him. I asked the nurse if he would please see me as a walk in.  She said he wouldnt see me at all because i had been discharged, even though he was just steps away from us:(.  She said i could write a letter to him if i wanted and gave me a pen and paper.  I wrote, that I was very sorry for putting his license and years of schooling on the line and that i would do whatever he wanted if given a second chance, or to please do this humanely and write my medications for one more month, so i have time to find another Dr.  That was yesterday. I have yet to hear from him:( . I asked the nurse if i could have my medical records and she said that they would fax them to my new? Dr. Otherwise she said they were going to charge me 1.50 !! a page. I have a thick folder of records, that would be a few hundred bucks that i dont have.
O, Im sooo screwed and scared. I can see myself spending the new year in the e.r.:(.  Actually i see them turning me away too, as ill now be labled a drug seeker ect.
Any one have any other ideas for me? I really dont know what to do.  I have meds till the 28th, 3 of those days ill be visiting my daughter for Christmas. One of the days is xmas. i dont have any time. I explained all this in my letter to the Dr.
I found a pain dr whos office is right at the end of my street, which would be great for me, they say that they will only take new patients upon referal <sp?. When i asked the nurse if he could give me a referal she said yea, no problem and wrote a couple of pain clinics on a piece of paper. She said just find a new Dr. and then call them (lol, yea right, you can hardly ever get through just to make your appt.) and they would send my records and a referal over to them.
WHAT DO I DO??

Dude,

HUGE miscommunications happen between the first line phone person and you and the Doc.  You said that you had a good relationship with the Doc.  Do whatever you have to do to get in front of him and speak with him.  If you have to show up at the the office and get his attention, do it.  Don't worry about being proper or polite.

Recent Example:  I went to see a different GP about something my GP of twenty years could not treat.  The new guy requested 5 years records from my old GP.  When I called my old GP for an appt., the phone person pretty much said my records were turned over to him and I was fired as a patient as I had another GP now....

I did not take this laying down.  I contacted my Docs wife and she said come right up and walk in to the treatment area.  Doc asked me if he was my GP or not.  Of course I said yes and explained the situation which he was good with.  All is good now.  Point being, don't let anyone run over you without a fight.

My pcp is my pm dr.. He was referred to me after ending up in the er because my pain was so bad. The er dr. sais he kinda catches people who fall through the cracks.  he was great to me. I had allready had problems with a couple drs. within weeks of getting discharged. One didnt understand why i would have pain because im a paraplegic...plus i was seeing a new dr. almost every visit. My original neuro doc had me on soooo many different drugs and soooo much morphine i could not take care of myslef. I let both those drs go.
Anyway my next question is.
I WOULD really like to talk with my Dr. but i dont know how to do it exactly. Its the type of office where you call the day before or day of your needed appt. to make an appt. Its a lil weird.  Since they called me allready to say i was being discharged. Will they let me make an appt.  I always had a good relationship with his nurse. should i just show up tomorrow and ask to please be seen as a walk in basis.
I really wonder what they found. i wonder if its possible that the nurse who called me decided to discharge me because she got results back and saw thc only. Would she have to discuss it with my dr?.  The reason i wonder is because the "drug" i took was garbage. Most people said it was 70% baby lax. It def should have been out of my urine.??
i know i messed up and i am more than willing to take weekly urine screens or even stop the mj....till i can figure out how to move to a M.M. state.
I live in No. FL. In a college town for drs. it is very hard to find a dr to even write the opioid i take because of its abuse. I have heard of lots of people who go down to So. fl to see drs. they get 3-4 times the amounts of meds that i do. Im sure if i went to one of THOSE docs that i could probably get anything i want. Considering im a t10 paraplegic.  I dont want to do that though, as i never have, because i was pretty happy with the help i was getting. i would say it was JUSt the right amount of meds i was on to be able to live a life and take care of myself 100% with a spinal cord injury.
Any thoughts/advice
Also if i did show up and he wont see me for some reason, am i entitled to all my medical records.
AND, how would i go about getting a copy of the drug screen so i know what im dealing with? When i asked the nurse she just said an illegal drug.

Hi,
  I am sorry your in such a bad postion. As you know it is so important to abide by the rules set forth by our PM Doctors. You will have a hard time finding a replacement but it is not impossible as there have been others here with similar problems and they have found new PM Doctors.
The first thing I suggest is getting a copy of your results so you know what your dealing with. If only the drug that your Doctor knew about has shown up then you should speak with him and ask for another chance. It is a huge problem for your Docotr as just knowing you may use an illegal substance and having it show up in test is two very different things. He now has paperwork that confirms use of an illeagal substance and legally he has to take action.
Make an appointment with your PCP and explain what happened and see if he can refer you to a new PMP.
I have had bloodwork come back negitive for my medications before and although the Doctor knew it was wrong he had to dismiss me due to a zero tolerence policy.
I am friends with his wife and so he knew I took my medications as prescribed but he had no choice but to dismiss me due to the zero tolerence be both agreed to by contract. He did find me a new PMP and he is awesome so things worked out in the end.
False negitives are so common and we have someone here asking about it all too often.
In your case you need to own up to your mistake and you will find a new PMP but there will be very strict rules for you until you can prove to the new Doctor he can trust you. You will have to have regular test and even suprise ones and maybe a pill count or you may only be prescibed a weeks worth of meds at a time.
Do not give up there is hope.

Hello,

I suggest that you try Drs. who specialize in pain management, not a regular MD.  Be straight up with them as to what's going on.  They will likely treat you.  The down side is that they they regularly do urine screens.  I don't think they would be cool with pot use but who knows if you discuss it with them.  Would your current Doc. give you another chance if you agreed to regular screens?  Just a thought.  Maybe something to throw out to him.....
