Hello,
I agree that this is a very personal decision.  I think that if you have completed your family and know that you don't want any more children, go ahead with the surgery.  I also have PCOS , but I'm only 25 and I want to give my 3 yr. old a brother or sister.  I wouldn't consider a hysterectomy and I wouldn't recommend one to anyone who wants more kids.  Something could be done.  One can lose weight (if any was gained due to lack of periods) and go on some treatments to conceive.  I also have dealt with hot flashes and such symptoms due to my missing period, but again, there is treatment available.  We just have to look for it.  I have not had a period in 3 yrs.  This is the second time I don't have it for a period of 3 yrs.  In between those 2 times, I had my son.  This time I also have complex cysts that will soon be removed.  If you're young, don't give up.  If you're sure you don't want to conceive, then do what you feel is right for you.  Sorry I made this long :)  Take care, and good luck with your decision.

This is an intensely personal decision.  I can tell you that I made the choice to have a hysterectomy and that I made them take my ovaries too because if you don't have ovaries, you don't have PCOS.
I had suffered for almost 20 years with PCOS.  It did not matter what weight I was at or what treatment they tried, I was miserable or could not tolerate the rx and still had evil pain with my periods.  Every cycle I had only 1 week of feeling normal, otherwise my horomones were out of control and, as such, so was I.  I was so miserable and it was affecting all areas of my life.  I honestly had friends willing to go with me to the doctor to petition for the hysterectomy I so desperatly wanted.  Finally, my doctor agreed to perform the surgery.
Because I never had a child, they had to perform the surgery by going in at about the bikini line.  It took six long weeks to recover, but every day I knew that the pain would be reduced and I would never have to go through that awful monthly pain again.
That was 7 years ago and I have never regretted the decision.  I would definitely do it again.  I use an estrogen gel that I rub on my arm nightly to postpone the menopause symptoms.  My engine takes a little longer to rev up than it use to, but that is a small price to pay to regain my life.
