Hi,
Vasodilation refers to dilation of the blood vessels. Most short-term anal fissures can heal with home treatment in 4 to 6 weeks. And home treatment involves sitting in warm water (sitz bath) for 20 minutes 2 or 3 times a day, increasing fiber and fluids in the diet, and using stool softeners or laxatives. However, if the fissures do not heal with these remedies or persists after 6 weeks, further evaluation by your doctor and additional treatment are indicated. Nitroglycerin cream can reduce the pressure in the internal anal muscle and allow the fissure to heal. Use it as prescribed by your doctor since this can cause side effects such as headaches, lightheadedness, or fainting from low blood pressure. Try to wear gloves when applying the nitroglycerin cream or wash your hands right after. If you use one cream, you can refrain from using the other. If your wedding is in three months time, it is best that you check with your doctor for proper management. The treatment will depend on the direct evaluation of your doctor. Take care and regards.

Hi,
Thank you for your update. How are you? Zinc oxide is indicated for treatment and prevention of rash. This is used widely and try not to worry. Nitric oxide, on the other hand, can cause vasodilation, hence, can also be used. In your case, you can continue with your zinc oxide as prescribed. Take care and regards.

Hi,
Xylocaine,as you mentioned from your other post,  indeed aids in numbing the area. A topical hydrocortisone preparation applied to the folds of the anal verge several times a day may help relieve symptoms and aids the healing process. While chemical sphincterotomy has been attempted using  nitric oxide and botulinum toxin to relieve ischemia and pain. It has also  been shown that a local application of topical nitrates reduces anal sphincter pressure and improves anodermal blood flow.  Talk to your doctor about these options. Almost 50 percent of fissures heal by themselves but the  longer that a fissure has persisted over time, the less likely it will be to heal by itself. Good anal hygiene consisting of a thorough cleansing after each bowel movement with thick quilted baby wipes may also help. Take care and best regards.

Hi,
Thanks for your update. I'm sorry to hear what you have been through. Unfortunately, the stronger anorectal gels and ointments  available are usually prescribed by doctors and can not be bought over-the-counter. Continue the conservative measures mentioned and I recommend that you check with your doctor for proper management. Take multivitamins also to help you stay strong despite this condition. Take care always.

Hi,
How are you? Conservative treatment for hemorrhoids include avoidance of straining, hot sitz bath, increased fluid intake and high fiber diet. There are hemorrhoidal creams available in the market that you can use. Aggressive therapy is reserved for patients who have persistent symptoms after 1 month of conservative therapy.   There are different stages for internal hemorrhoids:  Stage I - Internal hemorrhoids that bleed; Stage II – Internal hemorrhoids that cause bleeding and prolapse with straining but return to their resting point by themselves; Stage III - Internal hemorrhoids that bleed and prolapse with straining and require manual effort for replacement into the anal canal and  Stage IV - Internal hemorrhoids that do not return into the anal canal and are therefore constantly outside. Have this checked by your doctor for proper evaluation. Take care and do keep us posted.
