Bi-lateral Diaphragmatic Paralysis neurological

Just in last few months diagnosed, but pft's from 4 yrs ago show presence.  MEP 30% severe, MIP 50% severe, MVV 50%, at what levels will I be forced to decide about respiratory?  Pulmonary & Neuro Doc both state no treatment for me, also have Hypopituitarism, 2ndry Adrenal Insufficiency, undiff connective tissue disease. No major difference now in sitting up vs supine. Major problem bending. 4yrs ago MVV was 75% now 50%, does this give me some idea of how long it will take to reach level of needing respirator?  Thank you.
