Chlamydia & gonorrhea

I am Male 42, from Pakistan I had unprotected sex  vaginal and slight anal penetration once (big mistake ) with a sex worker on 27 Sept 13, on 29 Sept I felt severe itching in my groins and irritation in anus. I took 750 mg Azithromycin 29  sept night followed by 250 mg Azithromycin with 400 mg cefixime on 30 Sept morning. I waited for 24 hours The symptoms didn't resolve rather I started having severe irritation after peeing. I waited 24 hours & I took 2 gram Azithromycin (four 500 mg tablets spread over 1 hour) on 1 Oct night.

It is now exact 48 hours most of the symptoms i.e. itching in groins and in anus, burning and irritation after peeing are 60% resolved. However, now I am having bladder discomfort and pain , urine urgency, irritation & pain after peeing (dysuria). In addition constant dull constant ache in right lateral side just above bladder (possibly) ureter.

I am at a place where no NAAT test are available except simple urine RE and culture. The symptomatic treatment is only option for me.

My questions is, after medication I have used am I covered for Chlamydia & gonorrhea & syphilis. How long should I wait for the symptoms to clears off.  Second after 48 hours of above medication  bladder and ureter discomfort and pain signify symptoms of which STD/STI.

Hi,
Yesterday after 7-8 days of initial medication.
- 750 mg Azithromycin 29  sept night followed by 250 mg Azithromycin with 400 mg cefixime on 30 Sept morning.
- 1st Oct 2 gram Azithromycin (four 500 mg tablets spread over 1 hour)
-4 Oct metronidazole 400 mg with cipro 500mg bid.

Symptoms : 5 October - bladder discomfort and pain , urine urgency, irritation & pain after peeing (dysuria). In addition constant ache lower abdomen which travel to bladder and groins. Pain was so intense on 4 -5 night that I could hardly sleep all night. No itching or discharge what so ever.

On 5 Oct yesterday visited a local doctor, no testing facility available here. He Prescribed rocephin IM 1 gram injection at 1300 hrs.
metronidazole 400 mg three times a day 7 days
Cipro 500mg bid - 14 days

On 5th Oct same day By 1700 hours pain , discomfort lower abdomen and bladder discomfort resolved by 80%.

Its now 24 hours post rocephin IM 1 gram injection, pain , discomfort lower abdomen and bladder discomfort resolved by 90% however severe irritation / cutting  pain in urethra not at tip but a bit back after urination and general discomfort in groins and testicles exist. No other symptoms

Question:
- 2-3 gram Azithromycin with 400 mg cefixime didn't work against my gonorrhea. Why?
-  Do I need more Azithromycin for chlamydia ?
-  Do I need to repeat injection after 24 hours for possible epididymis, gonoccocal UTI. urethritis
- Possibility of Urethral stricture as gonorrhea was not treated for 8 days

- 1 gram rocephin seem to work

Hi,

Its 7 days post exposure, In spite of above medications to cover (Gonorrhea & Chlamydia) . Today From last 6 hours I have constant lower abdomen pain on sides. Fell heavy and pronounced discomfort in bladder. Urine urgency and dysuria. Constant urethral discomfort.  No rash, red mark .No flu , fever or body / muscle aches. At present I have typical prostatitis  symptoms, however with itchy scrotum.

Today I have started metronidazole 400 mg with cipro 500mg Bid to rule out any E.coli or Trichomoniasis related prostatitis.

IRight now no medical facility near by and  have to wait for another 7 days to get expert medical help USG and tests etc.

Question:
The symptoms I have confuse me is it possible to have prostatitis 7 days post exposure?

Apart from been in Pain and discomfort I am freaking out.......

Thanks dear, are you sure symptoms can last for two weeks and is the medication I took enough to cover me?

If a STD symptoms can last up to 2 weeks, but you would need specific testing to see what is going on. Since you medicated you would have to wait 3-4 weeks for any kind of testing now.
