Hello Doctor --
I am a 33 year old heterosexual male. In mid-September 2007 I had protected vaginal intercourse with a dancer at a strip club (three separate instances during that evening; three separate condoms used). I have had no other sexual experiences/exposures since that time.
Approximately eleven days later, I was tested for chlamidya and ghonnorhea, and also had what was called a "recent exposure" herpes test (IgM test?). Each of those tests came back negative. At approximately eleven weeks after the incident with the stripper, I had an HIV test, a syphllis test, and a hepatitis test. Each of these tests also came back negative.
In late March 2008 (approximately 6.5 months after the incident with the stripper), I noticed a small (maybe 1-2 mm across) bump on my foreskin (I am uncircumcised). This bump has been present about 7 days; it has the appearance of a pimple, but without a "whitehead" or other such feature at the top. It is painless; does not appear to be fluid filled; has not crusted over; and does not appear to have increased or decreased in size. There is one such pimple (not multiple bumps or a clustered bunch of them), and its surface appears to be smooth, without a dimple or any kind of "texture" at the top (including no cauliflower-like appearance). This "pimple" or "bump" has a reddish/pinkish appearance, particularly around the base, and looks a bit like an inflamed Fordyce spot (of which I have many on the shaft of my penis and foreskin). Basically it looks like a raised, round bit of skin that has become inflamed, but which is painless.
Based on this description and my STD testing history, does this "bump" sound like it could be HPV or herpes, given the timing (i.e., its appearance approximately 6.5 months post-exposure) and the fact that it appears on my foreskin, which was covered with a latex condom during the exposure? Is it possible/likely that I could have contracted HPV on another unprotected part of my body (e.g., top of the thigh), and it transferred to my foreskin? Are there other types of STD testing that you would recommend? Is it possible/likely that what I've described could be an inflamed Fordyce spot or some other sort of non-venereal skin condition? Finally, if you recommend that I see a doctor, what type should I see -- dermatologist, STD doctor, etc?
