163 days after exposure

I'm a 35 year old male. Bacl on July 8, 2007 I recieved un-protected oral sex from an old female friend who stated that she was disease free. A week later I began to exprience pain and a burning sensation all over my penis. I went to the doctor for an exam and the Doc said that there was sign of infection, no discharge and no sores. The Doc took a urine sample and said that there was no red or white blood cells and no glucose in my urine. She told me to return if I delevoped discharge and sores. I did not but a couple weeks later I developed an ache in my right testes. I went back to the doctor and was referred to a Urologist. I had a Testicluar Ultra-sound and saw the Urologist on Sept 5th. I was examined by the Urologist and I gave a urine sample. The ultra sound and urine sample all came back negative. Fast forward to this past weekend (Dec 15-16) I'm getting that same pain and burning like sensation. I noticed a little round scar that's red underneath the head of my penis. It burns when I bathe and when I put peroxide or Neosporin on it. I did use Dail Men's soap with exfoliating beads on it. Do you think that pain and scar/lesion could be caused by the soap and vigourous washing or could it be an STD that has taken a while to show itself?

Herpes is the most common cause of genital sores.  Although the true initial outbreak always occurs within 3 weeks, some people have asymptomatic infections that ony start to cause visible outbreaks months or years later.  From your description, herpes seems unlikely, however.  And if that's what you have, you didn't necessarily catch it during the exposure you describe above.

If in doubt, return to your doctor.  I won't have any further comments or advice.

No STD causes the symptoms you describe.  In any case, receiving oral sex is low risk for STD.  Further, you have seen two providers who reassured you and found no evidence of STD and I agree.

As to other causes of your symptoms, the main thing that enters my mind is the chronic pelvic pain syndrome, which used to be considered a form or prostatitis but might not involve the prostate gland at all.  Google "chronic pelvic pain syndrome" for lots of interesting reading; I suggest starting with the Wikipedia article, which is excellent. (It probably will come up no. 5-6 on the google list.)
