Burning skin 9 weeks after oral

Hi Doctor,
On June 24th, 2011, I received unprotected oral sex from a sex worker.  It lasted a couple minutes & then I ejaculated outside of her mouth.  We then proceeded to a chair where she used her breasts on my penis, with her spit as lube, until I ejaculated again.  I then showered & cleaned myself w/ soap & I peed to clean out my penis.  When I got to my car a few mins later, I slopped & rubbed in h& sanitizer all over my penis in an attempt to kill any germs or viruses that might have been picked up.

Since then until now, I have suffered from some symptoms.  I have felt a slight to mild burning on my entire penis, it’s felt irritated, it has burned (ever so slightly) when I pee, & I have felt a kind of numb pain in isolated areas on the head.  All these symptoms have come & gone over that time until now.  Today, just the head feels irritated & I THINK I am seeing discolorations or very small bumps.  

I have never had any discharge & any other visual sign of an STI/STD, & the Doctors I’ve been to have dismissed everyhting as nothing to worry about.

I have been tested 4 times for Gonorrhea & Clammidiya, as well as syphalis & HIV.  All have come back negative.

On July 21st, I started using canesten cream.  I used it for 7 days & noticed no improvement.

On Aug 4, I was given Triamcinolone Acetonide / Neomycin, which I applied after a shower in the morning & then again in the evening.  I did this for 1 week, no improvement.

On August 21st, the Doctor gave me 2 grams of Zithromax, just in case, which I took that night.

It’s been 9 weeks and still feels slightly irritated in general all the time.


1)Can this be the onset of something (warts)?
2)Could the irritation have been caused by the hand Sanitizer or is it most likely from my encounter?
3) would the hand sanitizer have killed anything like HPV or Herpes?
4) Finally, does it sound like I am suffering from something that I might pass on to someone else is I have unprotected intercourse?

Welcome to the STD forum.

There is no STD that causes the sort of symptoms you describe.  They are, however, typical for genitally focused anxiety -- and it certainly is clear you are very anxious about the sexual events you describe.  That the symptoms are psychological is the best explanation why various medications have not helped.  To your specific questions:

1) HPV does not cause symptoms like this.

2) Since you describe no visible sign of irritation -- redness, rash, etc -- it is unlikely the sanitizer is the cause, any more than the encounter itself.  The lack of response to the antifungal and steroid creams is additional evidence against a physical cause.

3) Using sanitizer after sex has never been studied to determine whether it reduces STD risk.  Probably very little, if at all.  It's one of those things like washing with soap and water -- makes sense, and probably doesn't hurt, but whether it actually helps is unknown.

4) If I were in your situation, regardless of these symptoms I would continue unprotected sex with my wife, secure in the knowlege I had nothing I could transmit to her.

If your symptoms continue, I recommend a) do not treat with anything at all and b) see a dermatologist to judge whether there is some sort of physical irritation.  In any case, I wouldn't be worried at all about any STDs in this situation.
