possible HSV-2

Hi Dr.,

Ok, Oct 22, I had an intoxcitaed affair. We both had our clothes off. I had maybe 10 seconds unprotected sex. Once I realized this I stopped. The woman then proceeded to give me very aggressive oral, to the point it was uncomfortable. Then she rubbed the tip of my penis with her thumb so hard it hurt and I had to stop her. I did not have an orgasm. I do not have a good number so I cant get in touch with her, but she was not a prostitute.

That night the tip of my penis was very sore around the opening. The next day it was slightly off color(slightly red). Then it continued to get more red and sore but only aroung the opening.

After 10 days I visited my pcp. The tip of my penis around the opening was red and sore. He said it did not look like herpes and I had no other symptoms. He gave me 10 days of valatrex just to be safe. 2 weeks later I went back. I saw my pcp and a urologist later the same day. Neither seemed to think I had herpes. The redness and soreness had not gone away in fact may have been slightly worse. He told me to try lotrimin twice a day. The urologist proscribed 10 days of Cipro. I also had the 8 panel std screen, all negative. This was 29 days post occurance.

After 40 days I tested again for hsv-2 by a blood test Igg. The results were negative. Also my pcp provided me with a prescription antifungal.

Its now over 6 weeks. The burning is all but gone. the redness is almost gone.

My pcp does not think I have hsv-2. He said at 40 days I am in a 70% catagory and higher because I did not have symptoms. I read valatrex can distort the blood tests and or retard the development of antibodies.

What are my chances of having acquired hsv-2? What if anything do you suggest going forward?

Welcome to our Forum. This exposure and concerns over herpes are clearly worrying you (I reviewed your interactions on the community site).  The time has come however for you to accept that you did not get herpes from this exposure and move on from here.  I say this for several reasons.  these include:

1.  You have been repeatedly examined by your PCP related to the redness/irritation you noted.  He does not think this is HSV.  He is a trained clinician, thus while the appearance of herpes can be variable, his opinion is of much value.

2.  Your statistical risk is low.  You do not know that your partner had HSV and even if she did, your risk of infection from a single encounter is, on average, less than 1 in 100 and perhaps as low as 1 in 10,000.  In your case, given how brief your exposure was, your risk is likely even lower than than that.

3.  you have a negative HSV blood test. While the test does not detect all infections at 40 days, it does detect about 2/3 of persons with proven herpes have positive blood tests.  That you do not have positive blood test at this time is further evidence that you did not get HSV.

My advice.. time fro you to move on.  As grace has suggested, stop examining yourself so much, so often and so carefully.   EWH

You could be developing irritation ue to the antifungals, combined with continued manipulation if you are applying it vigorously.  

At 12 weeks (not 8) about 90% of persons with recently acquired HSV have positve antibody tests.  I see no reason for you to test, nor do I see any reason for this thread to go on. This will be the final answer. EWH

Dear Dr,

I am sorry to trouble with with more questions, but if you could share your opinions.

It is now almost 8 weeks post occurance. I still get burning and redness on the tip of my penis. I have been on antifungals for 3 and 1/12 weeks. Lotrim for 14 days,  a combo steriod for a week, and now Tinactin powder. I apply the powder morning and night. In the mornings there is no burn and only slight redness, by the afternoon with walking, work, etc. its red and burns. Overall it seems to be getting better but very slowly.
(1) Am I in danger of developing a tolerance to the antifungals?
(2) Is there anything else I could think of taking?

Also, as you know I am quite anxious. I am seeing someone for this. However, as far as the hsv2 potential exposure is concerned, you indicated that the last time I tested (40 days post occurance) I was in  2/3s catagory. Its now almost 8 week. I have read that if I test at roughly 8 weeks I will be in a 90% catagory.
(1) is that true?
(2) do you think I should?

Ultimately, I am not so much worried about me, as I would never want to possibly expose someone else.

Dear Dr. Hook,

Thank you for your answer. I guess I am frustrated by the red irritation around the opening of my penis for over 6 weeks now. Its slowly going away but it is still slightly red. The antifungal is helping, not as fast as I would like I guess.

I just figured it had to be herpes because, what else could it be. Some kind of stubon yeast or bacteria infection?
