Could use help

Ok heres my story last mid October i had unprotected sex with a girl i met 2 weeks prior. As soon as i got home i washed my genitals and even using hand sanitizer and even mixing it with water and washing my genitals including the head, a day or 2 later i notice a white discharge after i urinated which only lasted a day and after i saw it less and less of it appeared when i urinated. The next few days i experienced burning while urinating and felt discomfort in my testicles.  I went to my doctor, he said i might have got chlamydia and we did the swab test to make sure, which came back negative. So he prescribe me Azithromycin (Zithromax) in 2 500 MG tablets to take at once. Symptoms dissapeared within the next few days.

Ok fast forwarding to Late December,  I had sex with the same girl except this time i wore a condom,  I put it on before we did intercourse and she did not give me a bj. During the sexual intercourse the condom kinda felt loose but i always checked it to make sure it was on. I did not ejaculate in her and in fact it was probably all over within 30 minutes or so. When i went to the washroom to remove the condom i noticed blood on it which was hers and i carefully removed the condom. As soon as i got home i washed my genitals and used hand sanitizer around the base or where ever the condom didnt cover. a few days later i noticed itching on my meatus and Burning while i urinated and the discomfort in my genital came back except now i did not notice any discharge. I began to research on the internet read about herpes and other STD. I got so scared that i found this anti biotics that my dentist had gave me when i had a tooth pulled, i think it was tetracycline. I took maybe 250 mg of that. Of course i didnt feel any better and as soon as the holidays were over i went to my doctor. He again prescribe me  Azithromycin  2 500 mg tablets and this time i did a blood test which he claimed was a full STD blood test. Since the clinic didnt do any blood test there i had to go to a hospital with a form  he gave me to take over there. Basically the sheet had a bunch of boxes that indicated various blood test, he mark one that said LIPID PANELW/ REFLEX DLDL ( Tchol,Trig,HDL,calc.LDL or DLDL when Trig>400)  and he marked HIV-1/HIV-2 SCRW/REFLEXES, he also marked RPR(DX) W/REFLEX CONFIRM FTA amd in a section under Microbiology he marked CHLAMYDIA AND N. GONORRHOEAE W/REFLEX ID,DNA PROBE,ENDOCX OR M/URET. So i take the blood test a few days later i called and he said everything was good and since i had taken the antibiotics my symptoms disappeared over the next few days . About 2 weeks later i was experience kinda like a burning sensation on the head of my penis which only lasted about a day, regardless i went to my doctor again and he said it was a yeast infection, again i asked him about the test results  he said everything came back negative and i asked him what about HSV he said that was negative also. He prescribe me Nystatin ointment USP 100,00 units per gram 30g (1.1 oz) so i  i applied the ointment all over my penis head and on my meatus, i kinda started to get some discomfort on my urethra i assumed it was the ointment getting inside but i didn't experienced the burning sensation on my penis again. The ointment felt kinda greasy and oily and after applying it for almost 2 weeks i stopped, but then 2 days later i noticed that my  penis head had some white cheezy looking stuff on it so i applied it again only to stop a week later. I didn't applied the ointment for about a week but decided to continue until i finished the tube.

Soo yea  here are my questions

1. What exactly did i had in those 2 past events my doctor really didnt say much about it?
2. How is it that i caught what i think is an NGU the second time with a condom on?
3. Any chance this can be herpes? i have not had any blisters or warts of any kind
4. What exactly was all those things written on my blood test form? On the corner it says Quest Diagnostic

Any suggestions?

Thanks

Oops forgot about that - it could be a yeast infection.

What about the yeast infection?  I have read that Antibiotics have yeast infection as a side effect?

My instinct says this is from using the hand sanitizer all over yourself.  Your genital skin is far too sensitive, and those sanitizers aren't made for the genital area.  (They also won't work to stop an std infection, by the way.  Once you're infected, you're infected.)

You had cholesterol, HIV, syphilis, and gonorrhea/chalmydia testing done.
